摘　　要：目的　研究幽门螺杆菌 (Hp)根除治疗对p5 3蛋白表达的影响 ,探讨Hp与胃癌的关系。 方法　利用免疫组化染色对 2 0 6例胃粘膜活检组织 ,包括正常胃粘膜、Hp阴性胃癌前各阶段病变、Hp阳性胃癌前各阶段病变Hp根除治疗前后及胃癌 ,进行 p5 3蛋白表达的分析。 结果　正常胃粘膜未见 p5 3蛋白表达。Hp阳性胃癌前阶段病变 p5 3蛋白表达阳性率显著高于正常胃粘膜及Hp阴性胃癌前阶段病变 ,其中Hp阳性肠上皮化生 p5 3蛋白表达阳性率显著高于Hp阴性相应病变组 ,低于Hp阳性异型增生组。Hp阳性胃癌前阶段病变Hp根除治疗后 ,其 p5 3蛋白表达阳性率显著降低。结论　Hp根除治疗后 ,胃癌前阶段病变p5 3蛋白表达阳性率明显降低 ;Hp感染可促进突变型 p5 3基因表达增强。? Objective To investigate the effect of the eradication of Helicobacter pylorei (Hp) on p53 protein expression,and understand the relationship between Hp and gastric carcinoma.Methods p53 protein immunohistochemical staining were used in the research.Total 206 gastric mucosal biopsy specimens,including normal gastric mucosa,Hp negative gastric precancerous lesions,Hp positive gastric precancerous lesions before and after eradication of Hp and gastric carcinoma,were studied.Results p53 protein positive experession was not found in normal gastric mucosa.Positivity of p53 protein expression in Hp positive gastric precancerous lesions was significantly higher than that in normal gastric mucosa and in Hp negative gastric precancerous lesions;Positivity of p53 expression in Hp positive metaplasia was significantly higher than that in Hp negative metaplasia,and lower than that in Hp positive dysplasia;After eradication of Hp,positivity of p53 experession significantly decreased in Hp positive gastric precancerous lesions.Conclusions After eradication of Hp,positivity of p53 experession in gastric precancerous lesions significantly decreased.It was supposed that Hp infection caused mutated p53 gene expression to increase. 〔
