摘　　要：目的:检测非酒精性脂肪肝(NAFLD)患者血清细胞角蛋白片段(CK-18 M30)的水平,探讨其与患者生化指标的相关性。方法:采用ELISA法检测71例NAFLD患者血清CK-18 M30的水平并与44例健康人比较。同时比较其中6例接受水飞蓟宾或多烯磷脂酰胆碱治疗好转前后患者血清CK-18 M30的水平。结果:NAFLD组患者血清CK-18 M30的浓度为(282.374 3±184.784 5)U/L,明显高于健康对照组的(100.088 8±32.482 8)U/L(P<0.001)。通过腹部B超结合患者的临床症状及生化指标将71例患者分为轻度组、中度组、重度组,其血清CK-18 M30的浓度分别为(148.640 6±83.109 4)U/L、(278.633 3±142.630 2)U/L与(471.264 9±256.265 7)U/L,差异有统计学意义(P<0.001)。NAFLD组患者血清CK-18 M30的水平与血清中丙氨酸转氨酶呈正相关(r=0.449,P<0.001)。6例治疗好转的NAFLD患者治疗后血清CK-18 M30的水平([265.084 0±130.040 0)U/L]低于治疗前([435.906 5±243.338 9)U/L],但其治疗前后差异没有统计学意义(P=0.160)。结论:NAFLD患者的血清CK-18 M30水平明显升高,对于判断NAFLD的严重程度具有一定价值。Objective： To investigate serum fragmented cytokeratin 18（CK-18 M30） levels in patients with nonalcoholic fatty liver disease （NAFLD）. Methods： Serum CK-18 M30 levels were measured by ELISA in 71 patients with NAFLD and 44 healthy persons. In addition, the levels of serum CK-18 M30 in 6 patients who improved after received Silibinin or Polyene Phosphatidyl choline were investigated. Results： Compared with healthy control, serum CK-18 M30 levels of patients with NAFLD were significantly higher （P〈0.001）. The 71 patients with NAFLD were divided into mild, moderate and severe three groups by abdominal B-mode ultrasound, clinical symptoms and biochemical parameters. The levels of CK-18 M30 in above groups were （ 148.640 6±83.109 4） U/L,（278.633 3±142.630 2） U/L and （471.264 9±256.265 7 ） U/L respectively, there were statistically significant difference （P〈0.001）. In the group of patients with NAFLD, the CK-18 M30 levels were positively correlated with serum alanine aminotransferase levels（r = 0.449, P 〈0.001）.Serum CK-18 M30 levels patients who improved after treatment significantly decreased. But there were no difference of serum CK-18 M30 levels before and after the treatment （P=0.160）. Conclusion： Serum CK-18 M30 levels in NAFLD patients are significantly higher. It has a certain value for judging the severity of NAFLD.
