颞下颌关节紊乱病患者关节液MMP-2、Hyp水平变化及意义

摘　　要：目的观察颞下颌关节紊乱病(TMD)基质金属蛋白酶-2(MMP-2)、羟脯氨酸(Hyp)水平变化及意义。方法 94例(105侧)TMD患者,临床检查和影像学检查诊断为关节炎症30侧(炎症组)、结构紊乱42侧(紊乱组)和骨关节病33侧(关节病组),另选20例健康体检者作为对照组,采用双抗体夹心酶联免疫法检测各组关节液标本中MMP-2和Hyp的水平。结果炎症组、紊乱组、关节病组的MMP-2及Hyp水平均明显高于对照组,紊乱组、关节病组的MMP-2及Hyp水平均明显低于炎症组,P均<0.01。结论 TMD患者关节液中MMP-2和Hyp水平存在差异,两指标联合检测有助于判断TMD病理损害程度。Objective To investigate the changes and significance of the matrix metalloproteinase-2（MMP-2） and hydroxyproline（Hyp） level in synovial fluid of patients with temporomandibular joint disorder（TMD）.Methods Total 94 samples were collected from 105 of TMD patients,which were divided into three groups according to the clinical examination and image analysis： 30 samples of arthritis inflammation patients（inflammation group）,42 samples of internal derangement（derangement group）,33 samples of osteoarthritis（osteoarthritis group）,then 20 of healthy volunteers were collected as the control group.The level of MMP-2 and Hyp in TMD synovial fluid was determined using Enzyme-linked immunosorbent assay.Results The level of MMP-2 and Hyp in the inflammation group,derangement group and osteoarthritis group were significantly higher than those in the control group,the level of MMP-2 and Hyp in osteoarthritis group and derangement group were significantly lower than those in inflammation group（all P〈0.01）.Conclusions The level of MMP-2 and Hyp of TMD patients were discrepant,and the combined detection of two indexs is beneficial to diagnose the degree of pathological lesion of the TMD.
