妊娠合并卵巢肿瘤的临床诊治分析

摘　　要：目的分析妊娠合并卵巢肿瘤的发生率、病理类型、诊断及处理方法。方法回顾性分析2009年1月1日—2011年1月1日解放军总医院收治的经临床诊断的32例妊娠合并卵巢肿瘤孕妇的临床资料,并对诊断及处理方法进行分析和总结。结果妊娠合并卵巢良性肿瘤的发生率为0.78%,妊娠合并卵巢恶性肿瘤的发生率为0.05%。妊娠合并卵巢良性肿瘤中,以妊娠合并成熟性畸胎瘤比例最高为60.00%,其他依次为浆液性囊腺瘤为16.67%和黏液性囊腺瘤为13.33%。结论妊娠合并卵巢肿瘤多为良性,以妊娠合并成熟性畸胎瘤比例最高。妊娠合并卵巢良性肿瘤时,若无蒂扭转或肿瘤破裂,肿瘤直径<5cm,严密监测下,可期待至足月治疗,妊娠合并卵巢恶性肿瘤,应视情况个体化治疗。Objective To analyze the incidence,pathology type,diagnose and therapy method of ovarian tumor accompanied by pregnancy in our hospital.Methods Retrospectively the clinical data of 32 cases of ovarian tumor accompanied by pregnancy in our hospital during 2009 to 2011.Results In our hospital,the incidence of ovarian tumor accompanied by pregnancy were benign tumor 0.78%,ovarian malignant tumor 0.05%.In the ovarian tumor accompanied by pregnancy,the proportion of mature teratoma is 60% and serous cystadenoma 16.67% and mucinous cystadenoma 13.33%.Conclusions Most of ovarian tumor accompanied by pregnancy is benign and the proportion of pregnancy complicating mature teratoma is maximum.In ovarian tumor accompanied by pregnancy,if have no torsion of pedicle and tumour rupture and diameter of the tumour is smaller than 5cm,at the careful monitor,it can expected to full term,if it malignant ovarian tumor accompanied by pregnancy,it should be individualized treatment according the condition.
