大承气汤联合低分子肝素治疗急性胰腺炎的随机对照试验

摘　　要：目的探讨大承气汤联合低分子肝素治疗急性胰腺炎的效果。方法将81例急性胰腺炎患者随机均分为A组(予低分子肝素)和B组(予大承气汤联合低分子肝素)。观察2组治疗前,治疗后第1,3,7天时的血白细胞计数、血淀粉酶和动脉血氧分压,治愈时间,转手术率、重症化率、病死率和总有效率。结果治疗前和治疗后第1天时2组对改善白细胞计数和血氧分压方面均无显著性差异,但在治疗第3,7天时B组的疗效优于A组;B组的治愈时间和总有效率优于A组;2组在血淀粉酶变化、转手术率、重症化率、病死率方面无显著性差异。结论大承气汤联合低分子肝素治疗急性胰腺炎,可显著降低白细胞计数,提高总有效率,缩短病程。但鉴于研究的局限性,建议行大样本、高质量的临床随机对照试验进一步研究。Objective It is to explore the therapeutic effect of Dachengqi decoction combined with low molecular weight heparin（LMWH） on acute pancreatitis（AP）.Methods 81 AP patients were randomly divided into group A treated with LMWH only and group B treated with Dachengqi decoction＋LMWH.WBC count,hemodiastase change,and p（O2） were determined before treatment and at 1,3,7 days after treatment in both groups.The curative days,aggravation rates,operation rates,mortality rates and total effective rates were also compared between the two groups.Results On the day before therapy and one day after treatment,there was no significant difference in WBC count and p（O2） between the two groups,but on the third day and seventh day,the curative effect of gorup B was better than that of group A.The curative days and total effective rates in group B were superior to group A.There was no signficant difference in the hemodiastase change,vaggravation rates,operation rates,mortality rates between the two groups.Conclusion The treatment with Dachengqi decoction combined with LMWH for AP can obviously decrease the WBC count,improve the total effective rates,shorten the curative days,but because of the limitation of the research,the large samples and high quality randomized clinical controlled trails to take further study are suggested.
