文拉法辛与氟西汀治疗迟滞性抑郁症对照研究

摘　　要：目的通过与氟西汀治疗对比,探讨文拉法辛治疗迟滞性抑郁症的疗效及安全性。方法将70例迟滞性抑郁症患者随机分成两组,每组35例。文拉法辛组剂量从75m g/d起始,7~10d加至150~225m g/d;氟西汀组剂量从20m g/d起始,7~10d内加至60 m g/d,治疗总疗程两组均为8周。在治疗前、治疗后1,2,4,8周末分别用汉密尔顿抑郁量表（HAM D）和副反应量表（TESS）评定疗效和不良反应情况。结果 8周治疗结束,文拉法辛组和氟西汀组的有效率分别为88.57%和85.71%,显效率分别为68.57%和60.00%,两组差异无统计学意义（χ2=0.057,P〉0.05）。不良反应两组差异无统计学意义（P〉0.05）。但在治疗的第1周,文拉法辛组HAM D总分明显低于氟西汀组,两组间差异有统计学意义（t=4.23,P〈0.05）。文拉法辛在治疗阻滞症状方面优于氟西汀。结论文拉法辛与氟西汀疗效和不良反应相当,但起效更快。Objective To explore the efficacy and safety of venlafaxine and fluoxetine in the treatment of retarded depression patients. Methods A total of 70 patients were randomly divided into two groups in which research group（n=35） took orally venlafaxine and control group（n= 35） did fluoxetine for 8 weeks. The initial doses of venlafazxine versus fluoxetine were 75rag versus 20 mg daily,and increased gradually to 150-225 mg/d versus 60 mg/d in 7-10 days. Efficacies were assessed with the Hamilton Depression Scale（HAMD）and adverse reactions with the Treatment Emergent Symptom Scale（TESS）before treatment and at the ends of the lst,2nd, 4th, 8th week. Results After 8 week treatment,effective rates were respectively 88. 57% in the research group and 85.71% in the control group,and obvious effective rates were 68.57 % vs 60.00% respectively. The efficacies and obvious effective rates of both groups were equivalence （X2= 0. 057,P〉0. 05）. The HAMD scores of research group lowered more significantly than control group in t week after treatment（P〈0.05） ,and the efficacy of venlafazxine in treatment of retarded depression was more effective than control group（t=4.23,P〈0. 05）. Conclusion Venlafaxine has a equivalent therapeutic efficacy and adverse effect rate comparing with fluoxetine,however,venlafaxine takes effects quicker than fluoxetine.
