精神分裂症性激素水平的影响因素分析

摘　　要：目的探讨精神分裂症性激素水平的影响因素。方法选取符合条件的精神分裂症患者95例,使用利培酮治疗,疗程6周,分别于治疗前和治疗后进行雌二醇(E 2)、孕酮(P rog)、睾酮(T)、催乳素(PRL)、促卵泡激素(FSH)、黄体生成素(LH)检测和BPRS评定。结果 6种性激素水平与精神分裂症患者的年龄、受教育程度、婚姻、家族史、病程、本次病期、BPRS总分及其因子分均有相关性。其中,男性患者治疗前仅E 2水平与家族史正相关(r=0.641,P=0.00),与BPRS总分负相关(r=-0.538,P=0.00),P rog水平与BPRS总分正相关(r=0.360,P=0.012),其余4种激素与BPRS总分无相关性;治疗后仍只有E 2水平与家族史正相关(r=0.587,P=0.00),与BPRS总分负相关(r=-0.564,P=0.00),P rog、PRL、FSH、LH水平与BPRS总分正相关(r=0.717、0.438、0.337、0.325,P=0.00、0.00、0.019、0.024)。女性患者无论治疗前还是治疗后,仅PRL水平与婚姻和家族史正相关(治疗前后均为r=0.964、0.887,P=0.00),其他性激素均与BPRS总分或因子分相关。结论精神分裂症的年龄、受教育程度、婚姻、家族史、病程、本次病期以及精神症状的类型和严重程度对性激素的水平均有影响。Objective To investigate the sex hormones＇ influencing factors of schizophrenia. Methods A total of 95 eligible pa tients with schizophrenia took Risperidone 6 weeks, and conducted progesterone （Prog）, testosterone （T）, prolactin （PRL）, folliclestimulating hormone （FSH）,luteinizing hormone （LH）measurement and BPRS assessment before and after treatment of estradiol （E2）. Results The levels of six sex hormones in patients with schizophrenia were correlated with age,education level,marital status, family history ,course of disease, this stage of disease,BPRS total score and factor scores. To male patients,before treatment,only E2 level was positively related to family history（r= 0. 641 ,P= 0. 00）,and negatively related to BPRS scores（r=-0. 538,P= 0.00）,Prog level was positively related to BPRS scores （r = 0. 360, P = 0. 012）, the other four hormones were no connection between BPRS total scores after treatment,only E2 level was positively related to family history （r = 0. 587,P= 0.00）and negatively correlation with BPRS total scores （r=-0. 564, P = 0.00）, Prog, PRL, FSH, LH levels were positively related to BPRS total scores （r= 0. 717,0. 438,0. 337,0. 325 ;P= 0. 00,0. 00,0. 019,0. 024）. To female patients,no matter before or after treatment ,only PRL level was positively related to marriage and family history （r= 0. 964,0. 887;P = 0. 00）,other sex hormones were associated with BPRS total scores or factor scores. Conclusion Schizophrenic age,education level,marital status,family history,course of disease, this stage of disease ,the type and severity of mental symptoms have effects on the levels of sex hormones.
