摘  要:目的:探讨多层螺旋CT应用于人左肺上叶段级与亚段级肺动脉成像研究的可行性,观察测量三维重建段级与亚段级肺动脉的管径。方法:选取行胸部多层螺旋CT增强扫描的正常成人340例,运用容积、再现、最大密度投影方法进行三维重建并观察测量左肺上叶段级与亚段级肺动脉,再按性别分2组、按年龄段分3组(20-40岁、41-60岁、61-87岁)进行分析比较。结果:肺动脉管径在性别之间、不同年龄组之间的差异都具有统计学意义;不同重建方法各有其优越性,最大密度投影重建图像能较好显示段级与亚段级肺动脉,应用此法测量较为准确。结论:基于多层螺旋CT增强扫描图像三维重建能较好显示并评价各级肺动脉,还可进行大样本测量研究,为临床肺部疑难病的诊断提供理论数据。Objective: To explore feasibility of 3D reconstruction of the segmental and subsegmental pulmonary artery of the superior lobe in left lung adopting multilayer spiral CT and observe and measure caliber. Methods: Totally 340 healthy adults underwent contrastenhanced CT angiography of thoracic part using multilayer spiral CT. Different methods of 3D reconstruction were used to observe and measure the caliber of segmental and sub-segmental pulmonary artery of the superior lobe of the left lung. Statistical analysis were carried out among all cases which were divided into two groups by gender and three groups by age (20-40 year, 41-60 year, 61-87 year). Results: There were significant differences in the pulmonary ar- tery caliber between male and female and among age groups. Different reconstruction methods had their own advantages. MIP and STS-MIP reconstruction can be better for showing segmental and subsegmental pulmonary artery, which were more accurate. Conclusion: 3D reconstruction based on enhanced scanning image of multilayer spiral CT can be better for displaying and evaluating each segmental of pulmonary artery. Large sample measurement also can be undertaken, which provides theoretical data for clinical diagnosis of pulmonary difficult and rare case.
