摘　　要：目的 检测P14ARF和半胱氨酸蛋白酶3（caspase-3）蛋白在子宫内膜腺癌（EC）、子宫内膜不典型增生（EIN）、正常子宫内膜中的表达,探讨P14ARF和caspase-3基因在子宫内膜癌发生、发展过程中的可能作用及相关性.方法 采用免疫组化SP法检测60例EC、30例EIN、30例正常子宫内膜组织中P14ARF、caspase-3蛋白的表达,结合临床病理特征进行分析,并探讨P14ARF与caspase-3的关联性.结果 P14ARF在EC、EIN和正常子宫内膜的阳性表达率分别为56.7％、73.3％、90.0％,caspase-3在EC、EIN和正常子宫内膜中的阳性表达率分别为51.7％、70.0％、86.7％,子宫内膜腺癌P14ARF、caspase-3的阳性表达率显著低于正常子宫内膜组织,差异有统计学意义（P＜0.01）.P14ARRF与caspase-3的阳性表达仅与EC的组织分级相关（P＜0.01）.P14ARF与caspase-3蛋白之间的阳性表达在EC中存在明显正相关性（r=0.433,P＜0.01）.结论 P14ARF、caspase-3蛋白在EC、EIN和正常子宫内膜中的表达逐渐升高.在EC中,随着细胞分化程度由高到低,二者的表达逐渐减少,2个指标之间有明显的正相关性.2个指标的联合检测,对子宫内膜腺癌的早期诊断和判断细胞分化程度具有一定参考价值.Objective To detect the expression of the P14ARF and caspase-3 protein in endometrial adenocarcinoma（EC）,endometrial atypical hyperplasia（EIN） and the normal endometrium.To examine the relation between P14ARF and caspase-3 in the genesis and progression of endometrial carcinoma.Methods The expression of P14ARF and caspase-3 protein in 60 cases of EC,30 cases of EIN and 30 cases of normal endometrial tissue were detected by streptavidin-perosidase（SP） immunohistochemical staining.The clinicopathological characteristics of patiens and the relevance of P14ARF and caspase-3 were analyzed.Results The positive rates of P14ARF in EC,EIN and normal endometrium were 56.7%,73.3%,90.0% respectively.The positive rates of caspase-3 in EC and EIN and normal endometrium were 51.7%,70.0%,86.7% respectively.The positive rates of P14ARF and caspase3 in EC were significantly lower than those in normal endometrium （P 〈0.01 ）.The positive expressions of P14ARF and caspase-3 were only related to the organizational grade of EC （ P 〈 0.01 ）.The positive expressions between P14ARF and caspase-3 protein were of significant positive correlation in EC （rs =0.433,P 〈0.01 ）.Conclusions The expression of P14ARF and caspase-3 protein in EC,EIN and normal endometrium were gradually increased.In EC,the expression of both were gradually decreased with cell differentiation from high to low,showing a significantly positive correlation between the two proteins.The combined detection of P14ARF and caspase-3 protein has a certain reference value in early diagnosis of EC and cell differentiation.
