血浆蛋白质Z及凝血4项检测在2型糖尿病并发症早期诊断中的意义

摘　　要：目的研究血浆蛋白质Z(PZ)及凝血4项(PT、APTT、TT、Fg)在2型糖尿病及血管并发症中的变化,并探讨其临床意义。方法以30例健康人为正常对照组,随机选择2型糖尿病患者60例,将其分为有血管并发症组(30例)和无血管并发症组(30例),PZ用ELISA法检测,试剂盒由法国Stago公司提供,在Moded680酶标仪上扫描测定。用美国Coulter公司ACL Futura全自动血凝分析仪及配套试剂检测凝血4项,血糖采用Beckman公司全自动生化分析仪检测。结果 糖尿病组PZ水平明显低于对照组(P值<0.001),在糖尿病组中有血管并发症组的PZ水平明显低于无并发症组(P值<0.01),PT、APTT、Fg在糖尿病无血管并发症组与对照组比较有差别(P值<0.05),有并发症组与无并发症组比较有差别(P<0.01),TT在各组之间均无显著差别(P值>0.05),PZ与血糖水平在健康人和患者中均有明显负相关(P均<0.001)。结论血栓形成在2型糖尿病血管并发症的发生发展中起着重要的作用,定期检查糖尿病患者血浆中PZ水平及凝血4项对于及早发现高凝倾向,进而加以预防和治疗并发症具有一定的参考价值。Objective to study the alteration of proteinZ. PT,APTT,TT,Fg in type 2diabetes mellitus and type 2 diabetes mellitus with angiopathy., to explore its clinical significance and relations with glucose. Methods 30 cases healthy persons and 30 cases of type 2 diabetes mellitus （T2DM） Patients without angiopathy and 30 cases type 2 diabetes mellitus with angiopathy, the plasma levels of proteinZ（PZ） were measured by Moded680 instrument. PT, APTT, TT,Fg were measured by ACL Futura instrument, made in America Coulter company. Results the level of proteinZ was significantly decreased in groups of type 2 diabetes mellitus patients with or without angiopathy than controls（P〈 0. 001）. proteinZ was significantly decreased in Patients with angiopathy than those without angiopathy（P〈0.01）, the level of PT, APTT, Fg were significantly differences among the groups（P〈0.05）, the level of proteinZ has clearly negtive relationship with glucose（P〈0. 001）. Conclusion Thrombosis in type 2 diabetes with angiopathy occurrence and development play an important role,it is important to regular measure the levels of PZ and PT, APTT, Fg for preven tion and treatment in type 2 diabetic angiopathy.
