三角形红唇黏膜瓣在功能性面横裂整复术中的应用

摘　　要：目的探讨面横裂功能性整复手术口角重建的一种理想术式。方法选择12例单侧面横裂患者,采用患侧上唇天然口角点外侧的三角形红唇黏膜瓣反折插入下唇红唇形成口角,口轮匝肌进行功能性重建、皮肤切口行"Z"字成形术。结果 12例患者术创均一期愈合,术后口角逼真自然,颊部丰满,左右对称,微笑自然。结论采用上唇三角形红唇黏膜瓣反折插入下唇红唇形成口角,联合口轮匝肌功能性重建,皮肤切口"Z"字成形是一种可选择的面横裂修复术式。Objective To introduce a satisfactory method for flmctiunal reconstruction of transverse facial cleft, including recon- struction of lips＇ angle. Methods For the 12 cases,we adopt natural triangle upper lip vermilion mucusal flaps to reconstruct angle of lips combined with muscle functional reconstruction and Z-plasty of the skin,the natural triangle upper lip nmcosal flap was just beside the natural point of the affected side of the upper lip. Results All wounds healed by first intension in 12 cases by this operation meth- od. Cheeks kept plump and bilateral symetry, the new angles of lips looked like natural ones,and movements of smile were coordinate. Conclusions Functional reconstI^Jction of transverse facial clet~ with natural triangle upper lip vermilion mt,cosal flap combined with Z-plasty of the skin is a good method.
