上海地区代谢综合征与牙周健康状况的相关性分析研究

摘　　要：目的探讨上海地区成年人群代谢综合征(MS)患病率和牙周健康状况的相关性。方法连续整群抽取上海地区成年人群6 818名,采用体格检查、生化指标采集和口腔检查采集相关性资料。牙周健康状况分为3个等级:良好、中等和不良,采用Logistic回归分析牙周健康状况和MS的相关性。结果 MS的各指标中腰围(WC)、收缩压(SBP)和空腹血糖(FPG)在不同牙周健康状况等级的人群中差异有显著性(P<0.05)。甘油三酯(TG)和舒张压(DBP)在良好等级与中等和不良等级人群中差异有显著性(P<0.05)。高密度脂蛋白胆固醇(HDL-C)在不同等级人群中差异无显著性。单因素分析显示,随着牙周健康状况的改善,MS患病率逐渐下降(P<0.01)。在纳入校正年龄、性别或增加不良习惯和饮食习惯和文化背景进行多因素分析后,MS的患病率仍然随牙周健康状况的改善而减少(P<0.05)。结论牙周健康状况和MS有相关性,中等和不良等级牙周健康状况人群是MS的易患人群,改善这2组人群的健康教育和牙周健康状况,也可对MS进行健康干预。Objective To investigate the relationship between metabolin syndrome （MS） and periodontal health condition in Shang- Hai area to provide evidence for healthcare policy making. Methods A clustered sampling of 6818 subjects from ShangHai area was included. Questionnaires and physical examinations were completed to collect clinical data. Logistic regression was used to analyze rele- vance of periodontal health condition with MS. Results Significant differences were found in waist circumstance （WC） , systolic blood pressure （SBP） and fasting blood glucose （FBG） among participants with different oral health conditions. Significant differences also existed in triglyceride and diastolic blood pressure （DBP） between subjects with good and poor oral health condition （ P 〈 O. 05 ）. In single-factor or multivariate analysis, oral hygiene condition was negatively correlated with the prevalence of MS（ P 〈0.05 ）. Conclu- sions Periodontal health condition and MS are closely related. People with poor and moderate periodontal health conditions have higher risk of MS. Improvement of health education and periodontal health condition of the two groups can interfere with their MS.
