高扭矩植入种植体的近期疗效的对比研究

摘　　要：目的:对比高扭矩和常规扭矩植入的种植体在负重前骨吸收(MBR)水平和近期累积存留率的差异,探讨高扭矩植入种植体的可行性及并发症的预防。方法:以35例后牙区牙列缺损,接受种植修复的患者为研究对象,共有55枚Bio-Med 3i Osseotite Tapered Certain种植体被纳入研究。种植体按植入扭矩值(ITV)分入A、B组,A组15枚种植体,ITV≤50N.cm,B组40枚种植体,50N.cm≤ITV≤90N.cm。采用埋置术式完成手术,术后即刻平行投照根尖片确定种植体周骨水平的基线,术后3～5个月复查时,再次投照种植体的根尖片,对比骨水平变化,得到负重前种植体MBR数据。结果:种植体植入术后3～5个月,A、B组间MBR值无统计学差异,全部种植体于二期术后一个月内完成固定烤瓷修复,平均随访11个月,观察期内种植体存留率100%。结论:观察期内高扭矩植入种植体不会引起种植体周骨吸收加重,近期疗效满意,值得进一步观察其远期疗效。Objective： To compare the difference of implants,inserted with normal or higher insertion torque value（ITV）,on marginal bone resorbtion（MBR） before loading and early survival rates,discussing the feasibility and complication preventing on implants insertting with higher ITV.Methods： 35 patients with missed teeth at the posterior region were taken part in this research program,and a total of 55 3i（r）Osseotite Tapered Certain was divided into A or B group based on implants＇ insertion torque value（ITV）.Group A（15 implants）,the controlled group,ITV≤50N.cm;Group B（40 implants）,the experiment group,50N.cm≤ITV≤90N.cm.All implants accepted submerged surgery,and have been taken intraoral radiographs immediately after surgery and 3-5 month after surgery for MBR calculating.Results： There are no different between group A and B on MBR 3-5 months after surgery,all implants received fixed porcelain restoration 1 month after second stage surgery,within the observation period,no implants failed,the cumulatting survival rate is 100% in both groups.Conclusions： implants insertting with higher torque would not cause to worse MBR,has a good clinical result,and be worth to go on research on longer observation period.
