会阴部巨大良性间叶瘤的临床诊治(附例报告并文献复习)

摘　　要：目的探讨会阴部巨大良性间叶瘤的临床病理特点及诊断和治疗方法。方法阴囊、阴茎渐进性肿大并不能性交的患者2例,经B超、CT和SPECT检查提示阴囊、阴茎肿大待诊(性质待查),行肿块切除术,完整切除肿块送病检。结果病理报告:表皮厚度大致正常,真皮层显著增厚,胶原纤维增生伴玻璃样变,其内平滑肌、脂肪、神经组织均有纤维化和玻璃样变,有散在慢性炎性细胞浸润和多核巨细胞反应,血管周围炎症细胞灶状浸润,以淋巴细胞为主。免疫组化:CD34(+),CD68(+),SMA(+),Vimentin(+),S-100(+)。病理诊断为良性间叶瘤伴炎性肉芽肿。术后随访1年半和6个月均未见肿瘤复发,且都能满意完成性交。结论阴囊、阴茎巨大良性间叶瘤是少见的皮肤软组织良性肿瘤,确诊需依靠病理学检查,彻底手术切除肿瘤为首选治疗方案,且预后良好,复发率较低。Objective To explore clinical pathological features,treatment methods and diagnosis of perineum huge benign mesenchymoma.Methods Two patients with the progressively enlarging penis and scrotum and the incapacity of sexual intercourse were referred to the diagnosis of scrotum penis enlargement （the nature undetermined） by the examination of B-ultrasound,CT and SPECT.The mass were completely resected and sent to pathological examination.Results The results of pathology report were listed as below ：normal mass epidermal thickness,significant thickening of the dermis,collagen fibers hyperplasia with hyalinization including smooth muscles,fats and nerve tissues,scattered chronic infiltration of inflammatory cells and multinucleated giant cell reac- tion,perivascular inflammatory cells infiltrating in the focal shape of which the lymphocytes are the majority.Immunohistochemical results ：CD34 （＋）,CD68 （＋）,SMA （＋）,Vimentin （＋）,S100 （＋）.Pathologic diagnosis ：benign mesenchymal tumor with inflammatory granuloma.The follow-up of one and a half year and six months both showed no tumor recurrence,and satisfied sexual intercourse.Conclusions Penis and scrotum huge benign mesenchymal tumor was a rare skin and soft tissue benign tumor,whose diagnosis depended on pathological examination.The complete surgical resection was the best choice for therapy with good prognosis and low recurrence rate.
