摘 要:目的:研究HPV16/18、Hpa基因在宫颈癌组织中的表达及二者的相关性。方法:采用免疫组织化学方法对宫颈癌石蜡标本45例、CINⅢ25例、CINⅠ~Ⅱ20例、正常宫颈组织10例进行HPV16/18及Hpa基因检测。结果:①HPV16/18在宫颈癌组织、ClNⅢ、CINⅠ~Ⅱ、正常宫颈组织中阳性表达率分别为86.7%、76.0%、25.0%、10.0%。宫颈癌组织中HPV16/18表达和阳性表达率普遍高于CIN组织和正常组织(χ2=11.91,P<0.05;χ2=20.53,P<0.01)。Hpa蛋白在宫颈癌组织、CINⅢ、CINⅠ~Ⅱ及正常宫颈组织中的阳性表达率分别为75.6%、28.4%、21.4%和10.0%。宫颈癌组织中Hpa阳性表达率普遍高于CIN组织和正常组织(χ2=31.169,P<0.05;χ2=12.94,P<0.05)。Hpa的表达与宫颈癌的淋巴结转移有关。②在宫颈病变中HPV16/18感染和Hpa基因表达之间呈正相关(χ2=4.288,P=0.038,r=0.363)。结论:宫颈癌组织中HPV16/18及Hpa基因的表达提示二者在宫颈癌的病因学中起作用,Hpa可作为判断宫颈癌预后的参考指标。Objective:To study the expressions of human papillomavirus 16/18(HPV 16/18) and Hpa gene in cervical cancer tissue and their correlation. Methods:Immunohistochemical method was used to detect HPV 16/18 and Hpa gene in paraffin-embedded samples of 45 cases with cervical cancer,25 cases with cervical intraepithelial neoplasia(CIN) Ⅲ,20 cases with CIN I-II,and 10 cases with normal cervical tissue. Results:The positive expression rates of HPV 16/18 in cervical cancer tissue,CIN Ⅲ,CIN I-II,and normal cervical tissue were 86.7%,76.0%,25.0%,and 10.0%,respectively.The positive expression rate of HPV 16/18 in cervical cancer tissue was significantly higher than those in CIN Ⅲ,CIN I-II,and normal cervical tissue(χ2=11.91,P0.05;χ2=20.53,P0.01).The positive expression rates of Hpa protein in cervical cancer tissue,CIN Ⅲ,CIN I-II,and normal cervical tissue were 75.6%,28.41%,21.42%,and 10.0%,respectively.The positive expression rate of Hpa protein in cervical cancer tissue was significantly higher than those in CIN Ⅲ,CIN I-II,and normal cervical tissue(χ2=31.169,P0.05;χ2=12.94,P0.05).Hpa expression was related to lymph node metastasis of cervical cancer.There was a positive correlation between HPV 16/18 infection and Hpa gene expression in cervical lesions(χ2=4.288,P=0.038,γ=0.363). Conclusion:The expressions of HPV 16/18 and Hpa gene in cervical cancer reveal that they contribute to the etiology of cervical cancer,Hpa can be used as a reference index to predict the prognosis of patients with cervical cancer.
