口腔颌面部鳞癌颈淋巴清扫术后临床并发症的分析及对策

摘　　要：[目的]探讨口腔颌面部鳞癌患者在颈淋巴清扫术后出现的临床并发症及其治疗对策。[方法]2009年8月～2011年5月期间,某院口腔颌面外科共收治口腔颌面部鳞癌患者137例,回顾分析84例患者的临床资料,总结分析他们在颈淋巴清扫术后出现的临床并发症及其治疗对策。[结果]137例口腔颌面部鳞癌患者中,41例(29.9%)在颈淋巴清扫术后出现临床并发症,包括伤口感染13例(9.5%),副神经损伤9例(6.6%),涎瘘、面神经下颌缘支损伤、乳糜瘘各5例(3.6%),皮瓣坏死4例(2.9%)。经合适治疗后上述并发症的预后均较好。[结论]口腔颌面部鳞癌患者在颈淋巴清扫术后可出现多种并发症,但经及早治疗后仍然可控制。[Objective]To explore the oral and maxillofacial squamous cell carcinoma patients with cervical cleaning the postoperatie in clinical complications and treatment measures.[Methods]In August 2009-May 2011,137 patients with oral and maxillofacial squamous cell carcinoma were retrospectively analyzed,the clinical data of 84 patients were analyzed,and we summarized and analyzed them in the postoperatie cervical clinical complications and treatment measures.[Results]Among 137 patients with oral and maxillofacial squamous cell carcinoma,41 cases（29.9%）showed clinical complications after cleaning in cervical wound infection,including 13 cases（9.5%）with wound infection,9 cases（6.6%）with accessory nerveinjury damage,5 fistula（3.6%）cases with saliary fistula,jaw facial nerve damage and a margin of chylothorax,and 4 cases withnskin flap necrosis（2.9%）.The appropriate treatment of the complications of the prognosis was good.[Conclusion]The patients with oral and maxillofacial squamous cell carcinoma could showed various complications after cleaning the lymph nodes in neck,but early treatment still can be effect.
