面神经受压程度对微血管减压术治疗面肌痉挛疗效的影响

摘　　要：目的探讨微血管减压术后面肌痉挛缓解率与神经受压程度的关系。方法回顾性分析48例面神经严重受压(A组)和同期365例面神经受压不明显的面肌痉挛病人(B组)的临床资料,均行微血管减压术,并对比两组术后效果。结果 A组:术后面肌痉挛即刻完全缓解44例(91.7%),明显减轻2例,部分减轻2例,总有效率为95.8%;术后并发症3例(6.2%)。B组:术后面肌痉挛即刻完全缓解338例(92.6%),明显减轻18例,部分减轻6例,无效3例,总有效率97.5%;术后并发症19例(5.2%)。两组术后即刻完全缓解率、总有效率及并发症发生率差异均无统计学意义(P>0.05)。结论微血管减压术是治疗面肌痉挛的首选方法,面神经受压程度对微血管减压术后痉挛缓解率无明显影响。Objective To explore the relationship between compression grade of the facial nerve and remission rate of hemifacial spasm after microvascular decompression （MVD）. Methods Clinical data of 48 hemifacial spasm patients with facial nerve compressed seriously （group A） and 365 hemifacial spasm patients with facial nerve compressed unobviously （group B） were analyzed retrospectively. MVD was performed in both groups and the postoperative effect was compared. Results Group A： The immediate complete remission of hemifacial spasm was achieved in 44 patients （91.7%） postoperatively, significant relief in 2 and partial relief in 2. The total effective rate was 95.8%, and the postoperative complication occurred in 3 patients （6.2%）. Group B： The immediate complete remission of hemifacial spasm was achieved in 338 patients （92.6%） postoperatively, significant relief in 18, partial relief in 6 and no change in 3. The total effective rate was 97.5%, and postoperative complication occurred in 19 cases （5.2%）. There was no statistical significance in differences of the immediate complote remission rate, total effective rate and complication rate between the two groups postoperatively （P〉0.05）. Conclusions MVD is the first choice for hemifacial spasm, and there is no obvious correlation between compression grade of the facial nerve and remission rate ofhemifacial spasm after MVD.
