小儿急性白血病CD34抗原表达的临床意义

摘　　要：目的探讨儿童急性白血病CD34抗原表达的临床意义。方法采用流式细胞术对78例急性淋巴细胞白血病(ALL)及27例急性髓系白血病(AML)进行免疫表型分析。结果 78例ALL患儿CD34阳性率为43.6%,27例AML患儿CD34阳性率为29.6%,CD34+组与CD34-组在性别和初诊时白细胞计数比较差异无统计学意义(P>0.05);CD34+组与CD34-组在肝脾淋巴结肿大等髓外浸润表现、诱导治疗4周的缓解率上差异有统计学意义。结论 CD34抗原表达在临床特征上主要表现在髓外浸润,为化疗反应差的影响因素。Objective To analyze the clinical significance of CD34 antigen expression in children with acute leukemia.Methods Immune phenotype were analyzed by flow cytometry in 78 cases with acute lymphoblastic leukemia（ALL） and 27 cases with acute myeloid leukemia（AML）.Results The positive rate of CD34 was 43.6% in ALL cases and 29.6% in AML cases,respectively.There was no significant difference in sex,white blood cells at onset between CD34＋ and CD34-group,while significant difference existed in the complete remission rate and the extramedullary infiltration between the two groups in both ALL and AML patients.Conclusion CD34 antigen expression is associated with extramedullary infiltration and it indicates poor response to treatment.
