脾胃系疾病PRO量表的计量心理学特性考核

摘　　要：【目的】对脾胃系疾病PRO量表进行计量心理学考核,以考评量表测量结果的可信性、有效性。【方法】采集现场调查的201例脾胃系疾病患者和99例健康志愿者的数据,考核脾胃系疾病PRO量表的分半信度、克朗巴赫系数、内容效度、结构效度、区分效度和可行性。【结果】整个量表的分半系数为0.881,生理、独立性、心理和环境各领域的分半系数分别为0.846、0.855、0.646、0.722。整个量表的Cronbach'sα系数为0.943,4个领域的Cronbach'sα系数分别为0.917、0.853、0.739、0.795。各领域积分与总分的相关系数大于0.7。生理领域除早饱、腹中重坠感这2个条目外,各条目与其所属方面、领域的相关系数大于0.5;其余3个领域各条目与所属方面、领域的相关系数大于0.7。整个量表模型的拟合优度指数为0.950,4个领域的拟合优度指数分别为0.791、0.996、0.911、0.953。健康志愿者与脾胃系疾病患者总分及各领域得分的差异均具有统计学意义(P<0.01)。【结论】脾胃系疾病PRO量表是一个具有良好的信度、效度和区分度的疾病专用量表,可操作性强,可用于中医、中西医结合脾胃系疾病的临床疗效评价。Objective To assess the psychological characteristics of patient reported outcome scale for gastroenterological diseases（GED-PRO）,and to measure the reliability and effectiveness of GED-PRO.Methods The data of 201 gastroenterological diseases patients and 99 healthy volunteers were collected in a field testing.Split-half reliability,Cronbach＇s α coefficient,content validity,construct validity,discriminant validity and feasibility of GED-PRO were analyzed.Results Split-half reliability of GED-PRO was 0.881,and split-half reliability of physical,independence,psychological and environmental dimension was 0.846,0.855,0.646,0.722,respectively.Cronbach＇s α coefficient of GED-PRO was 0.943,and that of physical,independence,psychological and environmental dimension was 0.917,0.853,0.739,0.795,respectively.The correlation coefficients of each dimension score with total score were over 0.7.Except for the aspect of fullness sense in the early morning and abdominal heaviness,the correlation coefficients of each item in physical dimension with physical dimension were over 0.5.The aspects in other three dimensions had correlation coefficient over 0.7 with its dimension.Goodness-of-fit index（GFI） of GED-PRO was 0.950.CFI of physical,independence,psychological and environmental dimension were 0.791,0.996,0.911,0.953,respectively.The total score and the dimension scores in gastroenterological diseases patients differed from those in healthy volunteers（P〈0.01）.Conclusion GED-PRO scale is a reliable and valid disease-specific instrument,with good discrimination.It can be used for the evaluation of clinical effect of traditional Chinese medicine,and integrated Chinese and western medicine in treating gastroenterological diseases.
