原发性胆汁性肝硬化患者调节性T细胞及相关细胞因子与肝功能损伤的相关性研究

摘　　要：目的观察原发性胆汁性肝硬化(PBC)患者不同时期外周血CD4+CD25+调节性T细胞亚群表达及细胞因子IL-2、IL-4、IL-10、TNF-α水平,探讨其与肝功能损伤的关系。方法以流式细胞术检测PBC患者外周血CD3+、CD4+、CD8+、CD4+CD25+、CD16+CD56T淋巴细胞;以双抗体夹心酶联免疫吸附(ELISA)法检测外周血细胞因子IL-2、IL-4、IL-10、TNF-α水平;以20例健康体检者作为正常对照组。结果 PBC患者外周血CD4+CD25+T细胞比例显著低于正常对照组(P<0.05),CD4+、CD8+T细胞群与正常人相比无显著性差异,NK细胞比例显著高于正常对照组(P<0.05)。肝功能严重损伤的PBC患者外周血CD4+CD25+T细胞比例显著低于肝功能轻度损伤者(P<0.05),PBC患者外周血CD4+CD25+T的数量与肝功能损伤指标ALP、GGT呈负相关。PBC患者血清IL-2、TNF-α与对照组比较明显升高,肝功能严重损伤的PBC患者IL-2和TNF-α水平明显高于肝功能轻度损伤者,血清IL-2和TNF-α水平与肝功能损伤指标ALP和GGT呈正相关。结论 PBC患者外周血CD4+CD25+T细胞比例及细胞因子浓度与肝功能损伤密切相关,因此推测T淋巴细胞免疫功能及细胞因子的紊乱在PBC发病机制中起一定作用。Objective It is to observe the expression of regulatory T Lymphocytes subsets such as CD4^＋ CD25^＋,and levels of cytokine such as IL-2,IL-4,IL-10 and TNF-α in peripheral blood in patients with primary bilary cirrhosis（PBC） in different periods,and approach its relationship with liver injury.Methods Perpheral blood T Lymphocytes such as CD3＋,CD4^＋,CD8^＋,CD4^＋ CD25^＋,CD16＋,CD56 were detected by flow cytometry,the levels of cytokines such as IL-2,IL-4,IL-10 and TNF-α were detected by ELISA.20 healthy volunteers were selected as controls.Results The levels of CD4^＋ CD25^＋ T cells in perpheral blood of patients with PBC was significantly lower than control group（P〈0.05）;CD4^＋,CD8^＋ T cells population was not significant difference compared with control group（P〈0.05）;NK cell ration was significantly higher than control group（P〈0.05）.CD4^＋ CD25^＋ T cell ratio in PBC patients with severe liver injury was significantly lower than mild liver injury（P〈0.05）.The number of CD4^＋ CD25^＋ T cells of PBC patients were negative correlated with the indexes of liver injury as ALP and GGT.The levels of IL-2 and TNF-α of PBC patients were significantly higher than control group,and in PBC patients with severe liver injury were significantly higher than those with liver injury,serum IL-2 and TNF-α level were positively correlated with ALP and GGT.Conclusion CD4^＋ CD25^＋ T cell ratio and the cytokine of PBC patients is closely related to liver damage,therefore infer that the disorder of immune function of T lymphocytes and cytokines play a role in the pathogenesis of PBC.
