摘 要:目的探讨苦参碱局部应用对兔尿道狭窄形成的影响,为临床治疗尿道狭窄提供新思路、新方法。方法 39只成年雄性新西兰大白兔,用F13号小儿膀胱电切镜在球部尿道电凝一周制成尿道狭窄兔模型。术后随机分为高剂量苦参碱组(16只,500 mg/d)、低剂量苦参碱组(16只,50 mg/d)和生理盐水组(7只,10 mL/d),各组均进行逆行尿道灌注,10mL/(d.次),连续灌注4周。给药30 d后,行逆行尿道造影、尿道镜检、离体尿道流率测定,观察各组尿道狭窄形成的情况;免疫组化染色了解各组狭窄处尿道TGF-β的表达情况;HE、天狼星红染色观察尿道狭窄处的病理形态。结果高剂量苦参碱组和低剂量苦参碱组胶原纤维和生理盐水组之间未见明显区别,各组都发现大量致密的纤维结缔组织取代了正常的海绵体组织,二者相互交错,无明显的分界,且细胞外基质中胶原纤维增多,弹性纤维减少。3组之间的尿道狭窄程度、离体尿流率均无显著性差异。结论苦参碱局部应用不能有效防治兔子尿道狭窄的形成。Objective It is to evaluate the effects of matrine on inhibiting urethral stricture formation and give a new way for the prevention and treatment of urethral stricture in clinic.Methods 39 adult male New Zealand rabbits were performed transurethral electricoagulationo of the bulbar urethra,underwent videourethroscopy with a pediatric resectoscope(F13).Then they were equally randomized into three groups: the high dosage matrine group(n=16,500 mg/day),the low dosage matrine group(n=16,50 mg/day) and the control group(0.9% NaCl,10 mL/day).Matrine was given for 4 weeks starting at the 1st day after coagulation by retrograde urethra irrigation once a day.Then the condition of urethral stricture was estimated by retrograde urethrogramand video urethroscopy,urinary flow rate,HE,histochemical and Sirius Red staining.Results The collagen fibers in the matrine groups were similar to it in the control group.In every group cavernous body of urethra were replaced by a great quantity of fibrous connective tissue,with interlacing and no obvious demarcation each other.In extracellular matrix collagen fibers were much more than elastic fibers.It in the high dosage and low dosage matrine groups was obviously smaller than in the control group.There were no significant differences in the degree of urethral stricture and urinary flow rates between each two groups of all.Conclusion Matrine has no significant inhibiting effect on urethral stricture formation.
