AcrySof Toric人工晶状体治疗白内障并矫正角膜散光的临床研究

摘　　要：目的评估Acry Sof散光型人工晶状体(intraocular lens,IOL)矫正白内障患者术前角膜散光的临床疗效和旋转稳定性。方法收集我院眼科角膜散光>1.0D年龄相关性白内障患者65例(76眼),按随机化原则将患者分为A、B2组:A组29例(34眼)植入AcrySof Toric IOL(试验组),B组36例(42眼)植入AcrySof Natural IOL(对照组)。观察植入术后1周、1个月、3个月患者裸眼视力、最佳矫正视力、残余散光、IOL轴位旋转稳定性。结果术后1周、1个月、3个月裸眼远视力≥0.8者A组分别占82.4%(28/34)、91.2%(31/34)、94.1%(32/34);B组分别占31.0%(13/42)、35.7%(15/42)、38.1%(16/42),2组比较差异均有统计学意义(均为P<0.05);最佳矫正远视力≥0.8者,2组比较差异无统计学意义(P>0.05)。A、B2组术前、术后3个月角膜散光度数分别比较,差异均无统计学意义(均为P>0.05);术后1周、1个月、3个月2组平均残余散光度数比较,差异均有统计学意义(均为P<0.05)。术后3个月A组平均离轴旋转度<3°,其中95.3%术眼离轴旋转度<5°。远视脱镜率:A组为94.1%(32/34),B组为38.1%(16/42)。2组术后无明显不良症状及并发症发生。结论 AcrySof Toric IOL具有良好的散光矫正效果并具有良好的旋转稳定性。AcrySof Toric IOL与普通IOL相比可明显改善患者术后远视力,显著提高了术后脱镜率。Objective To evaluate the efficacy and rotational stability of AcrySof Toric intraocular lens（IOL） implantation on correct preexisting corneal astigmatism in cataract surgery.Methods Sixty-five patients（76 eyes） with age-related cataract in our hospital with corneal astigmatism greater than 1.0 D were randomly divided into two groups.Group A（experimental group） with 29 patients（34 eyes） received AcrySof Toric IOL implantation and group B（control group） with 36 patients（42 eyes） received AcrySof natural IOL.Uncorrected distant visual acuity（UCDVA）,best corrected distant visual acuity（BCDVA）,residual astigmatism and rotational stability of IOL at postoperative 1 week,1 month,3 months were observed.Results UCDVA with 0.8 or better than 0.8 at postoperative 1 week,1 month,3 months accounted for 82.4%（28/34）,91.2%（31/34）,94.1%（32/34） in group A and 31.0%（13/42）,35.7%（15/42）,38.1%（16/42） in group B,respectively,there were significant differences between two groups（all P0.05）,but there was no statistical difference in BCDVA with 0.8 or better than 0.8 between two groups（P0.05）.There were no significant differences in corneal astigmatism before operation and postoperative 3 months between two groups（all P0.05）.There were statistical differences in the mean residual astigmatisms between two groups at postoperative 1 week,1 month and 3 months（all P0.05）.The mean axis rotation in group A was less than 3 degrees at postoperative 3 months,most（95.3%） less than 5 degrees.Spectacles independence rate in group A and B were 94.1%（32/34） and 38.1%（16/42）,respectively.No postoperative complication was found.Conclusion AcrySof Toric IOL is effective in astigmatism correction with good rotational stability.Compared with normal IOL,AcrySof Toric IOL has indeal postoperative distant visual acuity and high spectacles independence rate.
