Duke＇s B期结肠癌患者钙粘附蛋白的表达与复发转移的关系

摘　　要：[目的]探讨钙粘附蛋白（E-cadherin）在Duke＇s B期结肠癌（colon carcinoma）中的表达情况及与根治术后复发转移的关系.[方法]采用SP免疫组化法检测80例结肠癌组织及癌旁正常结肠组织中E-cadherin的表达,并分析其与临床病理特征及复发转移的关系.[结果]E-cadherin的表达在结肠癌组织中明显低于癌旁正常结肠组织,且差异有显著性（P＜0.01）;E-cadherin表达与性别、年龄、浸润深度无关.不同分化组的E-cadherin表达有明显差异（P〈0.01）,分化越差,E-cadherin阳性表达率越低;同样的临床分期及病理,无转移组有81.97%（50/61） E-cadherin表达阳性,而转移组为36.84%（7/19）,两组间相比较差异有显著性（P＜0.01）.[结论]E-cadherin的低表达与结肠癌的侵袭性有关,E-cadherin表达阴性者术后更容易发生复发转移,可以作为判断预后的客观指标.[Objective] To explore the expression of E-cadherin in Duke＇s B colon carcinoma and its relation with the recurrence and metastasis after radical operation. [Methods] SP immunohistochemical staining method was used to detect the expression of E--cadherin in cancer tissue and adjacent normal tissue of 80 patients with colon carcinoma. The relationship between the expression of E-cadherin and clinicopathologieal features, recurrence or metastasis was analyzed. [Results]The expression of E-eadherin in colon cancer tissue was lower than that in normal colon tissue, and there was significant difference（ P 〈0.01）. The expression of E-cadherin had no correlation with sex, age and infiltration. There was significant difference in the expression of E-eadherin among various dif- ferentiation groups（ P 〈0.01）. If the differentiation was poorer, the positive rate of E-cadherin was lower. Positive rate of E-cadherin in non-metastasis group and metastasis group with the same clinical stage and pathology was 81.96 % （50/61 ） and 36.81 % （ 7 / 19）, respectively, and there was significant difference between two groups （ P 〈0.01）. [Conclusion] Low expression of E-cadherin is related with the invasion of colon carcinoma. Patients with negative expression of E-cadherin are apt to have the recurrence and metastasis. Therefore, E-cadherin may be the prognostic indicators of colon carcinoma.
