摘 要:目的观察舒芬太尼预处理对全脑缺血再灌注损伤大鼠脑保护功能的影响,初步探讨舒芬太尼预处理脑保护的机制。方法雄性SD大鼠40只随机分为五组,每组8只。假手术组(Sham组)、缺血再灌注组(I\R组)和不同剂量舒芬太尼预处理组(L、M组和H组)。缺血再灌注组(I\R组)和不同剂量舒芬太尼预处理组在缺血前分别泵入生理盐水、舒芬太尼0.5、1.5μg.kg-1和4.5μg.kg-1,持续泵注5min,暂停5min,重复3次(容积1mL/5min,预处理时间30min)。采用四动脉阻断法(pulsinelli-4vo法)致全脑缺血,15min后恢复再灌注。再灌注24h后取血并断头取脑,测脑含水量、脑组织总钙和血清S100β含量。结果与Sham组比较,I\R组脑组织含水量、脑组织总钙与血清S100β含量均明显增加(均P<0.05);与I\R组比较,舒芬太尼预处理三个剂量组脑含水量、脑组织总钙与血清S100β含量均明显降低(P<0.05);舒芬太尼预处理三个剂量组间比较,与M组比较,L组脑含水量、脑组织总钙与血清S100β含量均明显增加(P<0.05),H组脑含水量明显增加(P<0.05);H组与L组比较,脑组织总钙明显降低(P<0.05)。结论舒芬太尼预处理可以减轻缺血再灌注所致的脑损伤,1.5μg.kg-1组作用较显著,其机制与减轻"钙超载"有关。Objective To investigate the effects of sufentanil preconditioning on total calcium of brain tissue and S100β Protein in rats with cerebral ischemia-reperfusion,and discusse the mechanism.Methods 40 male SD rats were randomly divided into five groups with 8 rats each,sham group(group Sham),ischemia-reperfusion group(group I/R)and different concentrations of sufentanil preconditioning groups(group L,M,H).The rats in group I/R and sufentanil preconditioning groups received normal saline,sufentanil infusion int-ravenously at 0.5 g·kg-1,1.5 g·kg-1 and 4.5 g·kg-1 in the regime(3×5 min infusion with 5 min interval) before cerebral ischemia.Four-vessel occlusion was used to establish the global cerebral ischemia reperfusion model in the study groups.Rats were killed at 24h after reperfusion.Brain water,total calcium of brain tissue and S100β Protein were measured.Results Compared with group Sham,the brain water,total calcium of brain tissue and S100β Protein in group I/R significantly increased(P〈0.05);Compared with group I/R,the brain water,total calcium of brain tissue and S100β protein in different concentrations of sufentanil preconditioning groups significantly discreased(P〈0.05);Among different concentrations of sufentanil preconditioning groups,compared with group M,the brain water,total calcium of brain tissue and S100β Protein in groups L and group H significantly increased(P〈0.05);compared with group L,total calcium of brain tissue in group H significantly decreased(P〈0.05).Conclusion Sufentanil preconditioning can protect rat brain against ischemia-reperfusion injury,especially at the dose of 1.5 g·kg-1and the mechanism may be relevant to reducing calcium overload.
