青年肺癌和老年肺癌临床特点对比分析

摘　　要：目的分析探讨青年肺癌与老年肺癌临床特点，为青年肺癌的临床诊治提供参考。方法回顾性分析福建省立医院2000年到2010年经组织学或细胞学证实的住院肺癌患者临床资料，对青年肺癌和老年肺癌的临床特点进行对比分析。结果和老年组相比，青年组误诊率高（56．7％vs21．7％，P〈0．005），吸烟率低（34．0％vs48．0％，P=0．009），女性患者比例高（53．3％vs30．8％，P〈0．005）；两组患者首发症状均以咳嗽多见，腺癌为最多见病理类型，确诊时均以晚期多见。结论肺癌目前误诊率仍高，确诊时大多处于晚期，如何提高早期诊断仍至关重要。青年肺癌、女性肺癌的发病率有上升趋势，腺癌为目前最常见病理类型，肺癌危险因素除吸烟外，其他的危险因素值得重视，肺癌的转移率仍高，青年组与老年组的转移程度相当。Objective To investigate the clinical characteristics between the elderly （age≥60） and the younger （age≤40） patients with lung cancer, and provide the reference for clinical treatment of younger patients. Methods The clinical datas of lung cancer patients who were confirmed by pathology in Fujian Provincial Hospital from 2000 to 2010 were retrospectively analyzed to compare the clinical characteristics between the elderly and younger patients with lung cancer. Results There were more female （53.3% vs 30.80/00, P 〈0.005）,lower smoking rates （34.0%vs 48.0%, P =0.009）,higher the misdiagnose rate （56.7%vs 21.7%, P 〈0.005） in younger group when compared with the elderly group. Cough was the most frequent presenting symptom and adenocareinoma was the predominant cell type in both groups, and most were in advanced stages as they were diagnosed. Conclusions Currently, the misdiagnose rate of lung cancer is still high, and most patients are in advance stages as they are diagnosed. Therefore, it is important to make the early diagnosis as soon as possible. The incidence of lung cancer in younger and female is trending to ascend and adenocarcinoma is the predominant cell type. Other risk factors of lung cancer should be paid attention to excluding smoking risk factor. The metastasis of lung cancer remains high and the metastasis degree is comparable in younger and elderly.
