D型细胞周期蛋白在喉咽癌中的表达及临床意义

摘　　要：目的探讨D型细胞周期蛋白(Cyclin D)与喉咽癌临床生物学行为之间的关系。方法取喉咽鳞状细胞癌组织标本56例,正常喉黏膜14例,应用免疫组化方法检测Cyclin D在喉咽癌组织和正常黏膜中的表达,RT-PCR检测其mRNA表达。结果 CyclinD1在喉咽癌组织中的阳性表达率78.57%,显著高于正常黏膜,与T分级、N分级、3年复发率及5年生存率均相关;CyclinD2在喉咽癌组织中的阳性表达率32.14%,高于正常黏膜,与3年复发率相关;CyclinD3在喉咽癌组织中的阳性表达率53.57%,显著高于正常黏膜,与N分级、3年复发率及5年生存率均相关。从3种亚型均阳性、二阳一阴、一阳二阴到三者均阴性,3年复发率依次降低,5年生存率依次升高。RT-PCR证实CyclinD1、CyclinD3在喉咽癌组织中的相对表达量明显高于正常黏膜,CyclinD2结果未得到RT-PCR证实。结论 Cyclin D的3种亚型在喉咽癌的发生发展中可能共同作用,同期研究其亚型对喉咽癌预后有较高的参考价值。OBJECTIVE To investigate the relationship between D-type Cyclins and biological behavior of hypopharyngeal squamous cell carcinoma. METHODS Immunohistochemistry and reverse transcription polymerase chain reaction（RT-PCR）were used to determine D-type Cyclins expression and gene amplification respectively in 56 cases of hypopharyngeal squamous cell carcinoma and 14 cases of normal adjacent epithelium.RESULTS Positive expression of CyclinD1 in the tumorous cell was observed in 44 cases（78.57%）,which was significant higher than that in normal epithelium.And cyclin D1 immunoreactivity was significantly associated with tumor size,lymph node metastases,three-year recurrence rate and five-year survive rate.CyclinD2 immunoreactivity in the tumorous cell was observed in 18 cases（32.14%）,which was significant higher than that in normal epithelium and significantly associated with three-year recurrence rate.CyclinD3 immunoreactivity in the tumorous cell was observed in 30 cases（53.57%）,which was significant higher than in normal epithelium.CyclinD3 immunoreactivity was significantly associated with lymph node metastases,three-year recurrence rate and five-year survive rate.As for co-expression of different subtypes of CyclinD,the patients with CyclinD1＋/CyclinD2＋/Cyclin D3＋expression experienced the highest recurrence and the poorest overall survival.Along with the decrease of the number of positive subtype expression,the patients＇ three-year recurrence rate decreased gradually,and their five-year survive rate increased step by step.RT-PCR confirmed that the average Cyclin D1 and CyclinD3 mRNA level in patients with hypopharyngeal carcinoma was much higher than those in normal epithelium.There was no difference between CyclinD2 mRNA level in patients with hypopharyngeal carcinoma and normal epithelium.CONCLUSION Different subtypes of Cyclin D may play roles together in hypopharyngeal squamous cell carcinoma progression.Researchs of all subtypes atthe same time could predict the probable disease course
