超声造影和增强CT对肝血管瘤的诊断价值比较

摘　　要：目的探讨增强CT与超声造影（CEUs对肝血管瘤的诊断价值．从而提高肝血管埔的诊断水平。方法回顾性分析在本院同时进行上腹部CT多期扫捕和CEus检查的肝血管瘤患者61例。统计分析2种检查对肝血管瘤的诊断效率。并进一步分别比较增强CT对不同大小的瘤体的检出率及CEUS对脂肪肝患者及非脂肪肝患者肝血管瘤的检出率，并对其结果分别做了统计学分析。结果61例患者中，增强CT共发现52例，CEUS共发现54例，二者的差别无统计学意义（P〉0．05）。增强CT对于瘤体直径〉4Ccm的肝血管瘤完全检出；对于〈4cm的，直径越小检出率越低且其差别具有统计学意义（P〈0．05）。CEUS对于脂肪肝组患者的检出率为61．54％，对非脂肪肝组患者的检出为95．83％，差异具行统计学意义（P〈0．05）。结论增强CT和CEUS对于肝血管瘤的诊断各有优势，合理选择可以明娃地提离诊断准确率。Objective To study the value of intravenous contrastenhanced computed tomography （CT）and intravenous contrast enhanced uhrasonography（CEUS） in the diagnosis of hepatic hemangioma（HH）.Methods 61 cases of hepatic hemangiomas under went simultaneously contrast enhanced muhiphase CT sean and ultrasound（US） examination. The efficiency of the diagnosis for hepatic hemangiomas by CT and US were statistically analysed. The detecting rates for hemangiomas in different sizes hy CT,and by US in patients with fatty liver and non-fatty liver were compared statistically. Results 52 lesions（S2 cases）and 5,1 lesions（5,1 eases） were detected by CT and US respectively, there was no signfifcant difference tetween them（P〉0.05） in detecting rate. Ovet 4 em hepatic hemangiomas in diameter were all detected by contrast enhanced CT,but less than 4 em ones, the smaller in size, the lower detecting rate, the difference was stal istically significant （P〈40.05）. CEUS for the detecting rates of henmgiomas in patients with fatty liver group and non fatty liver group were 61.54 % and 95.8%espectively, the difference between them was statistically significant （P〈0.0S）.Conclusion Contrastenhanced CTaud uhrasonography in detection of hepatic hemangioma have their ownadvanta ges,a reasonable choice can significantly improve the diagnostic accuracy.
