5-FU与MTX对96例恶性滋养细胞肿瘤Ⅰ期行单药化疗的体会

摘　　要：目的探讨5-FU与MTX对恶性滋养细胞肿瘤Ⅰ期行单药化疗的疗效及其毒副作用的比较。方法回顾性分析并随访50例5-FU单药化疗和46例MTX单药化疗的恶性滋养细胞肿瘤Ⅰ期病例。结果 50例5-FU单药化疗病例均在一个疗程后至18日血hCG定量下降并达一个对数,并持续3周呈进行性下降,子宫病灶缩小或消失(B超证实),有效率100%,副反应:骨髓抑制22例,占44%;肛周、口腔粘膜损伤6例,占12%;消化道反应20例(限于呕吐、腹泻,未出现伪膜性肠炎)占40%;药物性肝炎2例(均为一过性药物性肝炎),占4%;脱发8例,占13.4%。46例MTX化疗病例中37例达到治愈标准,占80.4%;其余9例经第二疗程化疗或联合化疗均治愈,副反应分别是骨髓抑制32例,占69.6%;肛周口腔粘膜损伤6例,占13%;消化道反应38例(限于呕吐、腹泻,未出现伪膜性肠炎),占82.6%;脱发16例,占26.8%;药物性肝炎0。结论 5-Fu较MTX单药化疗疗效佳,毒副性反应小,相对更适合Ⅰ期恶性滋养细胞肿瘤的治疗。Objective Effect of 5-FU and MTX in malignant trophoblastic tumor stage line single agent chemotherapy efficacy and toxic side effect comparison.Methods Retrospective analysis and follow-up of 50 cases of 5-FU single-agent chemotherapy and 46 cases of MTX single-agent chemotherapy of malignant trophoblastic tumor stage I cases.Results In 50 cases of 5-FU single-agent chemotherapy cases were in after a course of treatment to 18 blood HCG quantization down and up to a logarithmic,and lasted 3 weeks descended,uterine lesions to shrink or disappear（B ultrasound confirmed）,with an efficiency of 100%,deputy reaction： inhibition of bone marrow in 22 cases,accounting for 44%,anal,oral mucosal injury in 6 cases,accounting for 12%,20 cases of digestive tract reactions（limited to vomiting,diarrhea,do not appear pseudomembranous colitis） accounted for 40%,2 cases of drug induced hepatitis（both as a sexual medicine hepatitis）,accounting for 4%,alopecia in 8 cases,accounting for 13.4% of.In 46 patients with MTX chemotherapy a case in 37 patients to achieve cure standard,accounting for 80.4%,the remaining 9 cases by second cycles of chemotherapy or chemotherapy were cured,side reactions were bone marrow suppression in 32 cases,accounting for 69.6%,perianal oral mucosal injury in 6 cases,accounting for 13%,38 cases of digestive tract reactions（limited to vomiting,diarrhea,does not appear pseudomembranous colitis）,accounting for 82.6%,alopecia in 16 cases,accounting for 26.8%,0 drug hepatitis.Conclusions 5-Fu MTX than single-agent chemotherapy efficacy,toxic and side reaction of small,relatively more suitable for stage I malignant trophoblastic tumor treatment.
