摘 要:目的明确磁共振波谱分析(MRS)测得代谢产物的比值与荧光强度定量的相关性,验证代谢与荧光强度定量、肿瘤恶性度是否相联系,为5-ALA荧光引导显微手术切除胶质瘤提高重要客观依据。方法对MRI检查怀疑恶性胶质瘤患者,应用Functiontool软件得到波谱图像及代谢物解剖图。对术前3小时患者口服5-ALA(20mg/kg),在Leica荧光显微镜辅助下切除肿瘤。应用荧光分光光度计测量各部分肿瘤标本荧光强度,得到肿瘤中聚光剂原卟啉Ⅸ荧光强度值。结果 MRS测得代谢产物比值与荧光强度定量显著相关。胶质瘤组织中荧光强度与Cho/Cr比值成正相关;与NAA/Cr比值成负相关。结论 1)胶质瘤肿瘤实质区的荧光强度明显高于近肿瘤组织和远肿瘤组织。2)MRS测得代谢产物比值与荧光强度定量显著相关,实现了代谢与荧光强度定量、肿瘤恶性度相联系。能够对5-ALA荧光引导显微手术切除胶质瘤前起到指导作用。Objective To test correlation between metabolite measured by magnetic resonance spectroscopy(MRS) and quantitative fluorescence intensity ratio,and to realize the possible relationship among metabolism,fluorescence intensity and malignant degree,to provide important objective basis for 5-ALA microsurgical resection of gliomas in fluorescence-guided technology.Methods The study was about MRI examination of patients suspected of malignant glioma.and metabolite spectral image anatomy of tumor parenchyma was testedusing.Functiontool software,3 hours before surgery in patients with oral 5-ALA(20mg/kg),intraoperative use of Leica fluorescence microscope images to remove the tumor.Fluorescence spectrophotometer measured the fluorescence intensity of tumor specimens obtained tumor agent protoporphyrin condenser fluorescence intensity.Results Glioma organization with fluorescence intensity of Cho/Cr ratio had a positive correlation;NAA/Cr ratio with a negative correlation.Conclusions 1)The fluorescence intensity of gliomas tumor parenchyma is significantly higher than near and far tumor area.2) Metabolites measured by MRS are significant correlated with quantitative fluorescence intensity ratio.It can realize the quantitative,metabolism and fluorescence intensity of associated malignant degrees and provide an important objective basis for 5-ALA fluorescence guide microsurgery resection of gliomas
