摘  要:目的:探讨肿瘤标志物甲胎蛋白(alpha-fetoprotein,AFP)、α-L-岩藻糖苷酶(alpha-L-fucosidase,AFU)、糖类抗原(CAl9-9)联合检测在原发性肝癌(primary hepatocellular carcinoma,PHC)诊断中的应用价值。方法:应用电化学发光免疫分析法和全自动生化分析仪法测定70例PHC患者和50例正常对照组血清AFP、AFU、CAl9-9的含量,并分析对PHC诊断的敏感性和特异性。结果:PHC组AFP、APU、CAl9-9水平明显高于正常对照组,差异极有统计学意义(P<0.01);AFP、AFU、CAl9-9单个肿癌标志物诊断PHC的敏感性分别为65.7%、87.1%、50.8%,特异性分别为92%、62%、78%;3种肿瘤标志物联合检测在PHC诊断中的敏感性和特异性分别为98.6%和98%,联合检测的敏感性与单项检测差异极有统计学意义(P<0.01)。结论:AFP、AFU、CAl9-9联合检测对PHC有一定的诊断价值,尤其可提高AFP阴性PHC的诊断率。Objective:To explore the application value of the combination detection of alpha-fetoprotein(AFP),alpha-L-fucosidase(AFU) and carbohydrate antigen(CAl9-9)in diagnosing primary hepatic carcinoma(PHC).Methods:70 cases of PHC and 50 cases of normal controls were detected serum AFP,AFU and CA19-9 by the electrochemiluminescence method and the fully automatic biochemical analyzer,and their sensibility and specificity to diagnose PHC were analyzed.Results:The serum levels of AFP,AFU and CAl9-9 in the PHC group were significant higher than those in the control group with statistical significance(P〈0.01).The sensibility of single AFP,AFU and CAl9-9 for diagnosing PHC were 65.7%,87.1% and 50.8% respectively.Their specificities were 92%,62% and 78% respectively.The sensibility and specificity of combined measurement of these three tumor markers were 98.6% and 98% respectively,showing statistical differences in the sensibility(P〈0.01).Conclusion:The combined measurement of AFP,AFU and CAl9-9 has some diagnostic value for PHC,and especially helps to raise the diagnosis rate of PHC with negative AFP.
