摘  要:目的:评价麻风在云南省文山州丘北县分子流行病学模式。方法:2002-2010年发现164例患者进入本研究,其中130份皮肤样本适用于可变数目串联重复序列(variable number of tandem repeat,VNTR)分型。收集患者人口统计资料,进行菌株VNTR分型和基因型的聚类分析。结果: 分析显示该县某些少数民族人群的发现率明显高于全国水平。聚类分析表明家庭内和邻近乡镇的菌株型相似,8年间某些聚集株持续出现。菌株基因型相匹配的病例所占百分率随地理分布而变化。北部乡镇聚集株的比例相当高,菌株分类仍与以前确定的组群一致。南部菌株基因型与北部不同,但无法鉴别南部菌株间的亲缘关系。结论: 社交活动,自然居住和职业环境等可能更利于北部菌株的传播。Objeelive: To evaluate the molecular epidemiological model of leprosy in Qiubei County, Wenshan Prefecture, Yunnan Province of China. Methods: One hundred and sixty - four patients detected from 2002 to 2010 were enrolled in the study, and 130 skin samples were suitable for variable number of tandem repeat (VNTR) typ- ing. Basic demographic information of the patients was collected, and VNTR genotyping, clustering and VNTR allelefrequencies were performed in order to reveal the trends and pattern in leprosy transmission in Qiubei. Results: The case detection rate among minority populations in the county was considerably higher than the national rate. Cluster analysis of allele frequencies showed similar strain types within family groups and neighboring townships. Some clus- ters of M. leprae continued to appear during the eight years. The percentage of cases showing near - matching geno- types varied with geo^aphy, showing a considerably higher rate in the No,hem townships of the county. The North- ern townships continued to show strain types falling into the groups previously defined. Southern genotypes were dis- tinct from those in the north, but kinship between swains was indiscernible in the southern parts of the county. Conclusion: Social activity, natural residence and occupational environments may be more favorable for the trans- mission of strains in the north.
