3D-OCT Direct View Method Enhances the Efficiency of Experimental Choroidal Neovascular Model

摘  要:目的探讨在3D光学相干断层扫描仪(3D-OCT)直视引导下,提高氪激光诱导兔脉络膜新生血管(CNV)模型成功率的可行性。方法青紫蓝兔20只随机分2组,选取右眼进行激光诱导CNV:A组为非3D-OCT引导组,B组为3D-OCT引导组。A组在每只兔视乳头下方2PD处氪激光光凝10点,出现气泡或少量出血即停止光凝。B组在眼底激光光凝的同时,3D-OCT同步扫描显示光凝点击穿Bruch膜为止。光凝21 d后行眼底照相、3D-OCT及眼底血管造影检查,随后处死动物,摘取眼球,观察光凝处视网膜组织学病理改变。结果光凝后21 d,A、B两组兔视网膜水肿基本消退、灰白色斑、出血斑周围见黄白色渗出物,均可见荧光素渗漏。A组64点出现Bruch膜破裂、45点出现CNV;B组100点Bruch膜破裂、86点出现CNV。两组比较差异有非常显著意义(P<0.01)。Bruch膜破裂与CNV出现率呈高度正相关。结论 Bruch膜的破坏是诱导CNV模型的关键,3D-OCT直视引导法可提高诱导兔CNV模型的成功率。Objective To establish a laser induced choroidal neovascularization (CNV) model by 3D-optical coherence tomography (3D-OCT di- rect view in chinchi!la rabbit. Methods Twenty chinchilla rabbits were randomly divided into 2 groups and right eyes of the rabbits were selected for CNV models :group A ( 10 rabbits, 10 eyes, non-OCT direct view method) and group B ( 10 rabbits, 10 eyes, OCT direct view method ). The ocu- lar fundus near to the optic disk 2 PD in group A was administrated by krypton laser-coagulation 10 spots until bubbles or minimal hem- orrhage emerged. Those in group B received a simultaneous OCT direct view to judge whether Bruch membrane had been disrupted be- sides all the operations in group A. Twenty-one days later, experimental eyes were examined with ocular fundus photo, OCT and angi- ography, histological changes also were examined. Results After 21 days, we found that retinal edema vanished, gray splash appeared and yellowish-white exudate emerged around the hemorrhage on the site of laser-coagulation, furthermore, we saw the fluorescein leakaging around laser-coagulation spots. There were 64 spots appeared Bruch~ membrane rupture and of which 45 spots appeared CNV in group A vs 100 spots and 86 spots respectively in group B. There was significant statistical difference by chi-square test (P 〈0. 01 ) and there had positive correlation between Bruch~ membrane rupture and CNV incidenee. Conclusions Bruch~ membrane rupture is the key of inducing CNV model. OCT direct view method can be used as an objective standard to enhance the efficiency of experimental CNV model.
