摘  要:目的探讨外伤眼硅油取出术前视网膜预防性和强化性光凝对预防取出油后视网膜再脱离的临床疗效。方法回顾性分析45例的临床资料,所有患者取出硅油前行视网膜赤道部环形(360°)预防性光凝和视网膜裂孔区进行强化性光凝。激光参数:光斑直径250—350μm,时间0.2—0.3s,光斑间距1-1.5个光斑,以出现三级光斑为准,光凝4—5排。结果硅油取出术后随访6—37个月,视网膜复位良好者42例,占93.33%。3例发生视网膜再脱离,占6.67%。其中1例因出现新的裂孑L而视网膜再脱离;另2例取油后脱离越过激光斑发生视网膜再脱离。视网膜再脱离的发生率明显低于文献报道的最高达25%以上者。结论硅油取出术前进行视网膜预防性和强化性激光光凝,可明显减少取硅油术后发生视网膜再脱离。Objective To evaluate the therapeutic effects of retinal preventive and strengthened laser photocoagulation before the extraction of silicone oil of eye trauma in preventing retinal detachment after the extraction. Method Retrospective study was conducted in 45 cases. All patients were treated with equatorial retina annular (360°) preventive laser and retinal holes strengthened laser photocoagilation. Laser parameter: spot diameter, 250 - 350 μm; time, 0.2 - 0.3 s; spot spacing is 1 - 1.5 spot, until reaching tertiary light spot. Laser photocoagulation was 4 - 5 rows. Results Following up for 6 - 37 months after the extraction of silicone oil, the retina reattached successfully in 42 cases (93.33%). retinal detachment occurred in 3cases (6.67%). One was influenced by the occurrence of new holes. The other two were attributable to the detachment passing laser spot. The incidence of retinal detachment was obviously lower than 25% by the reports in the literature. Conclusion Retinal detachment reduced significantly after the extraction by retinal preventive and strengthened laser photocoagulation before the extraction of silicone oil. It can improve the success rate of opermion efficiently.
