眼内容摘除61例原因及羟基磷灰石义眼座植入临床分析

摘　　要：目的分析行眼内容摘除的原因及羟基磷灰石义眼座I期植入的效果。方法眼内容摘除术联合I期羟基磷灰石义眼座植入61例。分析眼内容摘除的原因及义眼座植人和义眼片佩戴后的效果。结果61例中有48例（78．7％）为严重眼外伤，其中10例严重眼球破裂伤、9例外伤后眼内炎、29例严重眼外伤修补术后的并发症或后遗症（眼球萎缩、粘连性角膜白斑）；4例（6．5％）为感染性角膜溃疡；2例（3．3％）为绝对期青光眼；4例（6．5％）为眼部手术后并发症；3例（5％）为角膜葡萄肿。61例中55例（90％）为60岁以下的中青年患者。义眼座植入术后无感染等严重并发症。术后佩戴亲水凝胶软性义眼片者舒适度更好。结论行眼内容摘除义眼座植入术者主要为严重眼外伤者，尤其是眼球破裂伤者。义眼座植入效果良好。Objective To analyze the causes of evisceration, and to study the results and feature of hydroxyapatite（HA） implantation. Methods Retrospectively study the patients underwent the evisceration and with the primary surgery of HA implantation. Results In 61 patients with the operation of evisceration and HA implantation, there were： 48 （78.7 % ） cases of ocular trauma, such as severe eyeball rupture and endophthalmitis, 4 （6.5%） cases of infective corneal ulcer, 2 （3.3%） cases of absolute glaucoma, 4 （6.5 % ） cases of side-effect of surgery and 3 （5%） cases of corneal staphyloma. 55 cases were younger than 60 years old. All the patients had good results without severe side effect. Conclusion Most of the patients under- went with evisceration and HA implantation was severe ocular trauma such as the eyeball rupture. The safety during the traffic and working should be emphasized to avoid and reduce the severe ocular trauma. The results of severe ocular trauma should be improved.
