钼靶、高频超声等方法联合应用筛查早期乳腺癌的结果评价

摘　　要：目的总结2006年5月至2011年5月乳腺筛查癌的特点,分析钼靶和超声的诊断特点,为筛查工作的继续开展提供指导。方法统计筛查癌的年龄分布、手术方式、病理类型、肿瘤大小和淋巴结转移数目,并与非筛查癌进行对比。计算钼靶和超声诊断的敏感性和特异性。结果筛查癌的年龄分布呈正态分布,高峰为40～60岁。筛查癌和非筛查癌相比,肿瘤小(2.4cmVS3.0cm,P=0.045),≤2cm的肿瘤比例大(56.3%VS31.1%,P<0.01),有腋窝淋巴结转移者比例小(9.4%比27.3%,P<0.05)。超声的敏感性和特异性高于钼靶(93.8%比53.1%,78.9%比77.3%),但两者互相补充,联合使用时将提高检出率。结论乳腺癌筛查可以早期发现乳腺癌,筛查的重点人群为40～60岁的女性。联合使用钼靶和超声进行筛查比单独使用更有效。Objective To summarize the characteristics of the breast cancers detected during the screening from 2006 to 2011.To analyze the features of mammography and ultrasonography diagnosing.The conclusion is used to direct the following screening work.Methods Analyzing the age distribution,operation mode,pathology,size,and the number of metastatic lymph nodes of the tumor.Comparing these characteristics with those of the non-screening breast cancers.Calculating the sensitivity and specificity of the mammography and ultrasonography.Results The age distribution of the screening cancers is a normal distribution.The peak is 40~60 years old.Compared to non-screening cancers,the size of the screening cancers is smaller（2.4cm to 3.0cm,P=0.045）,the proportion of tumors whose diameters ≤2cm is larger（56.3% to 31.1%,P0.01）,and the proportion of tumors with metastatic lymph nodes is smaller（9.4% to 27.3%,P0.05）.The sensitivity and specificity of Ultrasonography is higher than those of mammography（93.8% to 53.1%,78.9% to 77.3%）,more cancers were detected when they were combined.Conclusions The breast cancer screening can detect breast cancers earlier.The screening work should focus on the females 40 to 60 years old.Combining mammography and ultrasonography works more effectively.
