摘 要:目的 总结用自行研制的脑胶质瘤放疗囊,对恶性胶质瘤进行瘤腔间质放疗的长期疗效。方法 在理论计算和模拟测定的基础上,对25例恶性胶质瘤患者在手术切除肿瘤后的残瘤腔内放置放疗囊,术后经皮穿刺向化疗囊内注入740~1 110 MEq/次的^(125)I,对残瘤细胞进行长时间、局部高剂量的近距离照射。结果 模拟测定放射源1~2cm范围内的吸收剂量为30.4~91.3cGy/h,是常规经颅照射剂量的3~5倍。本组病例生存期明显延长,Kamofsky积分提高。病理证实近放射源处无肿瘤细胞增殖。结论 利用放疗囊对胶质瘤进行的^(125)I近距离放疗是一种无辐射污染、增减放射源简便,可对局部残瘤进行长周期、高剂量放疗的新方法。Objective To introduce our experiences in the treatment of malignant gliocytomas by the self-designed radiother-apeutic sacs, in which the ' I was used as the source of radiotherapy. Methods Totally, 25 patients with malignant gliocytoma were treated post-operatively with the radio-therapeutic sacs which were direcdy placed within the cavities after tumor removal. The dose of 125 I injected into a sac was 740 - 1110 MEq each time, thus the remnant tumor cells possibly in the tumor cavity would be instantly irradiated near by the source of radiation. Results Analogue test showed that the absorbed dose of I 1 ~ 2 cm around the radio-source was 30.40 - 91.30 Gy/h, about 3-5 times higher than that through trans-skull irradiation. The average life-span in this series of patients was obviously prolonged. The patients' Karnofsky scores were increased. Pathological examination indicated that no cancerous cells were found 3 cm around the source of irradiation. Conclusions The advantages using the radio-therapeutic sac as a measure for prevention of recurrence of tumor were prominent. The dose and the irradiation time in the course of treatment could be adjusted according to conditions of the patient.
