摘 要:目的 探讨多药耐药基因(MDR1)和多药耐药相关蛋白基因(MRP)在食管癌、贲门癌中表达的临床意义。 方法 采用逆转录-多聚酶链反应(RT-PCR),对29例食管癌、贲门癌癌组织及癌旁组织中MDR1和MRP的表达进行检测。 结果 癌组织中MDR1阳性率为65.5%,高于癌旁组织中MDR1的阳性率,为37.9%(P<0.05),癌组织MDR1信使核糖核酸(mRNA)表达水平也显著高于癌旁组织(P<0.01);癌组织的MRP阳性率为48.3%,高于癌旁组织(27.6%),但无差异(P>0.05),而癌组织MRPmRNA表达水平与癌旁组织比较则有差异(P<0.05);中、低分化肿瘤的MDR1和MRP表达阳性率增高,两基因的mRNA表达水平显著高于高分化肿瘤(P<0.05)。 结论食管、贲门癌具有内源性多药耐药性;MDR1和MRP表达与食管、贲门癌的组织学类型及TNM分期无关。Objective To investigate the clinical significance of multidrug resistance gene and multidrug resistance related protein gene expression in patients with carcinoma of esophagus and gastric cardia. Methods Using reverse transcription polymerase chain reaction (RT PCR),the gene expression of multidrug resistance gene and multidrug resistance related protein gene in 29 cases of esophageal and gastric cardiac cancer was determined. Results Positive expression of multidrug resistance gene was 65.5% in 29 cases with esophageal and gastric cardiac cancer,37.9% in tissues adjacent to the carcinoma site,and the difference was significant( P <0.05).The expression levels of multidrug resistance gene in cancer tissues was much higher than that in tissues adjacent to the cancer site( P <0.01).The positive rate of multidrug resistance related protein gene expression in cancer tissues was 48.3%,which was higher than 27.6% in tissue adjacent to the cancer site.The gene expression levels was still higher in comparison with tissues adjacent to the cancer site( P <0.05).Expression levels of multidrug resistance gene and multidrug resistance related protein gene in undifferentiated tumor was significantly higher than that of well differentiated tumor( P <0.05). Conclusion Esophageal and cardiac carcinoma had primary multidrug resistance gene.Multidrug resistance and multidrug resistance related protein gene expression was not related with the histological pattern of tumor,and cancer staging, but may reflect malignant biologic behavior of undifferentiated tumor.
