摘  要:目的探讨全自动毛细管电泳仪在血红蛋白分析中的临床应用价值。方法使用全自动毛细管电泳仪及配套试剂进行血红蛋白电泳(法国Sebia公司Capillarys2),检测了5088例标本,分析异常发生频率;选出229例同时进行毛细管电泳和地贫基因分析,比较其符合率。结果全自动毛细管电泳仪测定5088例标本,发现异常1042例,其中β地贫757例(HbA2>4.0%),HbF增高23例,异常Hb262例。229例同时进行毛细血管电泳与地贫基因检测比较,与β地贫符合率100%,与HbH病和HbCS的符合率分别是97.83%和95.00%。结论全自动毛细管电泳仪检测Hb电泳精密度及准确度较好,能够分离和量化出HbA2、HbF异常增高及HbE、HbH、HbBart's、HbCS等异常血红蛋白条带,可为地中海贫血及血红蛋白病筛查及诊断提供快速准确的诊断依据。Objective To assess the value of capillary electrophoresis system in the analysis of hemoglobin. Methods We applied the completely automatic capillary vessel eleetrophoretic apparatus and the necessary reagent to examine the hemoglobin electrophoresis in 5 088 example specimens, and analyze the frequency of abnormality. 229 examples were selected simultaneously for the capillary vessel electrophoresis and the thalassemia gene analysis. Then the coincidence rate were compared. Results Among 5088 sample, there were 1 042 abnormal samples. Among abnormal samples, there were 757 cases with β- thalassemia Hb As, 23 cases with high HbF, 262 cases with abnormal Hb. In 229 cases, the rate that agarose electrophoresis was consistent with gene diagnoses for β-thalassemia was 100%, the HbH disease and HbCS was 97.83% and 95.00%. Conclusion The accuracy and precision of this method can meet the requirement of clinical determination. Full automated capillary eleetrophoresis system can detect trace abnormal hemoglobin and accurately identifies and quantify the high hemoglobin fractions HbF and HbA2, and abnormal hemoglobin HbE, HbH, HbBar's and HbCS, hence help for the diagnosis and screening of thalassemia and some hemoglobin diseases.
