摘  要:目的:采用基于体素的形态学(VBM)方法探讨多发性硬化患者的脑灰质变化。方法:应用3.0T MRI,对25例复发缓解型多发性硬化(RRMS)患者和22例正常志愿者进行3D T_1WI扫描,通过基于体素的形态学分析全脑灰质和区域灰质核团的变化。结果:与正常对照组比较,多发性硬化的全脑灰质明显萎缩(679.40±52.68mlvs 629.09±54.50ml,P=0.040),其中双侧丘脑、双侧壳核、双侧苍白球、左侧尾状核、左侧海马、左侧海马旁回、双侧扣带回前部、右侧颞上回、右侧颞中回、双侧岛叶体积明显缩小(P<0.001)。结论:多发性硬化存在脑灰质萎缩,主要累及深部灰质核团和边缘叶系统。Purpose:To analyze the gray matter volume changes in multiple sclerosis patients by means of voxel - based morphometry.Methods:Three - dimensional T_1 weighted images were obtained from 25 patients with relapsing - remitting multiple sclerosis(RRMS) and 22 matched healthy controls on a 3.0T MRI machine.Voxel - based morphometry was used to analyze the atrophy of the whole and regional gray matter changes.Results:When compared with controls,the patients with MS showed the presence of significant gray matter atrophy(679.40±52.68ml vs 629.09±54.50ml,P = 0.040),and the volumes of bilateral thalami,bilateral putamen nuclei,bilateral globus pallidum,left caudate nuclei,left hippocampus, left parahippocampal gyrus,bilateral anterior cingulate gyrus,right superior temporal gyrus,right middle temporal gyrus and bilateral insula were significantly reduced(P0.001).Conclusion:Gray matter atrophy is exist with multiple sclerosis,and mainly involves the deep gray nuclei and limbic system.
