摘 要:目的:使用T_2WI-PWI研究淋巴结放疗后的早期变化。方法:10只新西兰白兔建立恶性淋巴结模型,给予一次性局部10Gy的放射性治疗,并于治疗前及治疗后的第1天、3天及7天采集磁共振灌注图像。由一位放射科医师绘制感兴趣区,生成时间-信号曲线,并计算半定量灌注参数:TTP、MSD%及PH%。此外,每次灌注图像采集完毕后取一只兔子行病理HE染色检查。结果采用单因素ANOVA分析。结果:放疗后,只有第7天的半定量灌注参数与治疗前比较存在统计学差异,TTP增加、MSD%减低(P<0.05)。放疗后第1天部分细胞核退变、细胞肿胀;第3天部分肿瘤细胞核碎裂,可见残余肿瘤细胞轮廓;第7天淋巴结内散在坏死。对侧肌肉的灌注参数在放疗时间点前后未见明显差异(P>0.05)。结论:磁共振半定量灌注参数可以在一定的程度上评估淋巴结放疗后早期的变化。Purpose:To explore the early response of metastatic lymph nodes after radiotherapy in animal models.Methods:The animal models of metastatic lymph nodes were made in 10 rabbits.The tumour masses and lymph nodes received single - dose radiotherapy of 10Gy locally.Perfusion - weighted dynamic susceptibility contrast(DSC) MRI was performed 1 day prior to as well as 1 day,3 days and 7 days after radiotherapy respectively.The time - intensity curves(TIC) of region of interest(ROI),of which was drawn by one radiologist,were generated automatically,and perfusion semi - quantitative parameters,such as time to peak(TTP),maximal slope descending(MSD%) and peak heighted(PH%) were calculated.The differences of these parameters,which were obtained before and after radiotherapy, were evaluated,and the results were analyzed with One - way ANOVA.HE staining was undergone for the pathological exam.Results:Only at seventh day after therapy,the TTP values were increased, and DSC%values were decreased,significantly differences were revealed(P 0.05)when compared with them obtained before therapy.Pathology results showed some stained nucleus and swollen cells at the first day after irradiation;at third day,some tumour cells were degenerated with necrosis and nuclear fragmentation;at seventh day,necrosis was scattered in lymph nodes.The muscle of contralateral limbs showed no statistic difference before and after the radiotherapy for the models(P 0.05).Conclusion: After radiotherapy,the semi - quantitative parameters,which are based on the TIC,can be used to observe early radiation response of malignant lymph nodes to some extent.
