TACE联合内皮抑制素治疗兔VX2肝移植瘤MRS特征与细胞凋亡的对照研究

摘　　要：目的:通过分析TACE联合内皮抑制素(ES)治疗兔VX2肝移植瘤后1 H-MRS与凋亡指数(AI)及细胞密度的关系,探讨1 H-MRS对治疗疗效评价的生物学基础。方法:24只新西兰大白兔采用直视下瘤组织块包埋法建立兔VX2肝移植瘤模型,随机分为TACE联合ES组、ES组和对照组。观察肿瘤治疗前后1 H-MRS所得的Cho峰值与Lip峰值及Cho/Lip峰值比的变化情况;免疫组织化学法及HE染色检测治疗后肿瘤组织AI及细胞密度,并将二者分别与1 H-MRS所得数据进行对比分析。结果:兔VX2肝移植瘤治疗后两个治疗组肿瘤组织Cho峰值及Cho/Lip峰值比降低幅度较对照组大,而Lip峰值升高幅度较大;移植瘤治疗后TACE联合ES组和ES组肿瘤组织AI值均高于对照组,TACE联合ES组肿瘤组织AI值高于ES组(P<0.05);TACE联合ES组细胞密度最低,ES组次之,对照组最高(P<0.05);AI与Cho峰值及Cho/Lip峰值比呈负相关(r=-0.552,P=0.018;r=-0.737,P=0.001),与Lip峰值呈正相关(r=0.710,P=0.009);细胞密度与Cho峰值呈正相关(r=0.701,P=0.011)。结论:TACE联合应用ES更容易促进肿瘤细胞凋亡,1 H-MRS能有效反映肿瘤治疗前后组织学改变。Objective：To explore the biological basis of 1H-MRS in the evaluation of the effect combination of transcatheter arterial chemoembolization（TACE） and endostatin（ES） on VX2 carcinoma implanted in rabbit liver by analyzing the relationship between 1H-MRS and cell density,apoptotic index of tumor after treatment.Methods：Twenty-four new zealand white rabbits with VX2 carcinoma models were established under direct surgical vision embedding tumor tissue,and randomly divided into three groups： control group,TACE combined with ES group and ES group.The changes of Cho peak,Lip peak and the ratio of Cho/Lip on the 1H-MRS before and after the treatment were analyzed.Apoptotic index and cell density were recorded according to immunohistochemistry and HE staining.1H-MRS and apoptotic index and cell density were analyzed.Results：After treatment,the Cho peak and the ratio of Cho/Lip of treatment groups decreased more dramatically than those of control group.The increased-amplitude of Lip peak was higher than that of control group.After treatment,the AI of the TACE combined with ES group and ES group was higher than that of control group,and the AI of TACE combined with ES group was higher than that of ES group（P0.05）;the cell density of the TACE combined with ES group was minimum,the control group＇s was maximum,and the ES group＇s was in the middle（P0.05）.Apoptotic index was correlated negatively with Cho peak and the ratio of Cho/Lip（r=-0.552,P=0.018l;r=-0.737,P=0.001）,and was positive to Lip peak（r=0.710,P=0.009）.Cell density was correlated positively with Cho peak（r=0.701,P=0.011）.Conclusion：1H-MRS could detect the histology change of the VX2 carcinoma implanted in rabbit liver after treatment,and combination of TACE and ES could promote the cell＇s apoptosis.
