面中部复合L形截骨缩窄术对上颌窦的影响

摘　　要：目的 探讨面中部复合L形截骨缩窄术开放上颌窦后的手术效果,以及对术后并发症产生的影响.方法 对2006年6月至2009年8月收治的67例患者行口内入路上颌骨颧骨颞骨复合体L形截骨缩窄术,其中52例患者分别于术前及术后12个月行CT检查,以GE AW 4.1软件做图像分析,并进行临床评估,观察患者上颌窦容积的改变、手术效果及并发症产生的情况.应用SAS 6.12进行单因素方差分析.结果 所有患者均对手术效果满意,图像分析显示：术前上颌窦容积左侧为（21 233.96±4 455.04） mm3,右侧为（22 020.64 ±3 663.82） mm3,术后12个月左侧为（17 840.91±4 381.03） mm3,右侧为（18 511.85 ±3 466.24） mm3,与术前比较,差异有统计学意义（P＜0.05）.术后无感染、牙髓坏死等并发症.结论 口内入路上颌骨颧骨颞骨复合体L形截骨缩窄术可明显改善患者外观,开放上颌窦后无明显并发症.Objective To investigate the result of zygomatic reduction with midface L-shaped osteotomy through intraoral approach. Methods From June 2006 to Aug. 2009,67 cases received zygomatic reduction with midface L-shaped osteotomy through intraoral approach.52 cases underwent CT scan before operation and 12 months after operation.The images were analyzed by software GE AW 4.1 for evaluation of clinical effect,maxillary sinus change and complication. SAS 6.12 software was applied for one-way ANOVA.Results Satisfactory results were achieved in all the patients.The volume of maxillary sinus at left and right side was （21 233.96 ±4 455.04） mm3,and （22 020.64 ± 3 663.82） mm3,respectively before operation： （17 840.91 ± 4 381.03） mm3 and （18 511.85 ± 3 466.24） mm3respectively 12 months after operation,showing a significant difference between them （P 〈 0.05 ）. No infection or dental pulp necrosis happened.Conclusions Good results can be achieved with intraoral L-shaped osteotomy for zygomatic reduction.Exposure of maxillary sinus would not cause any complication.
