彩色多普勒超声诊断卵巢卵泡膜细胞瘤

摘　　要：目的探讨彩色多普勒超声在卵巢卵泡膜细胞瘤诊断中的应用价值。方法回顾性分析25例经病理证实为卵巢卵泡膜细胞瘤患者的超声、临床表现与病理结果。结果超声检出肿块23例(23/25,92.00%),漏诊2例(2/25,8.00%);正确诊断13例(13/25,52.00%),误诊10例(10/25,40.00%)。20例(20/23,86.96%)呈实性弱回声,其中2例后方回声衰减;3例(3/23,13.04%)为囊实混合性包块。肿块内探及点线状血流信号或无明显血流。病理诊断为单纯卵泡膜细胞瘤8例(8/25,32.00%),纤维-卵泡膜细胞瘤16例(16/25,64.00%),颗粒-卵泡膜细胞瘤1例(1/25,4.00%)。结论卵泡膜细胞瘤超声表现有一定特异性;分析超声图像、临床表现及病理特点有助于提高对卵泡膜细胞瘤的诊断水平。Objective To investigate the value of color Doppler ultrasonography in the diagnosis of ovarian thecoma.Methods Ultrasonographic characteristics of 25 patients with ovarian thecoma confirmed with pathology were analyzed.Results Twenty-three（23/25,92.00%） masses were detected with ultrasonography and 2 were missed（2/25,8.00%）.Thirteen masses were diagnosed（13/25,52.00%） and 10 were misdiagnosed（10/25,40.00%）.Ultrasonography displayed that 20（20/23,86.96%） thecoma were solid,of which 2 were shown as a homogenous hypoechoic pattern with no posterior sound attenuation.Three（3/23,13.04%） appeared as a mixed pattern of solid and cystic components.The blood flow signal in the thecoma reduced or disappeared.In 25 patients,there were 8 ovarian thecoma（32.00%）,16 fibrothecomas（64.00%） and 1 granulosa-theca cell tumor（4%）.Conclusion Typical ovarian thecoma has specific ultrasonographic features.Combining analysis of ultrasonic image,clinical manifestations and pathological characteristics will help to improve the ultrosonographic diagnostic rate of ovarian thecoma.
