瑞舒伐他汀强化降脂治疗缺血性脑卒中的疗效

摘　　要：目的探讨瑞舒伐他汀强化降脂对缺血性脑卒中患者血脂和颈动脉粥样硬化斑块的影响。方法选择72例缺血性脑卒中患者,随机分为强化组和普通组,两组患者均予常规对症治疗(降颅压、控制血压血糖、抗血小板、改善脑循环等),普通组在此基础上加用瑞舒伐他汀10 mg qd口服,强化组加用瑞舒伐他汀20 mg qd口服,连用6个月。结果治疗6个月后,两组患者TC、TG和LDL-C水平均较治疗前明显下降、HDL-C水平较治疗前明显上升(P<0.05或P<0.01),且强化组下降或上升的幅度较普通组更明显(P<0.05);强化组斑块面积和IMT均较治疗前明显减小(P<0.05),而普通组治疗前后斑块面积和IMT无明显变化(P>0.05),治疗后强化组斑块面积和IMT较普通组明显减少(P<0.05)。结论瑞舒伐他汀强化降脂对缺血性脑卒中具有更强降脂作用,通过减少血小板的黏附和聚集,从而降低颈动脉斑块面积及IMT,阻断并逆转颈动脉粥样斑块的进展。Objective To evaluate the effect of rosuvastatin＇s intensive lipid-lowering function on plasma lipid and carotid atherosclerotic plaques in patients with ischemic stroke. Methods All of 72 cases of ischemic stroke patients were randomly divided into intensive group and general group. All patients were given conventional symptomatic treatment （reducing the intracranial pressure, control of blood pressure and blood glucose, anti-platelet, improving cerebral circulation, etc.）, The general group were given rosuvastatin 10 mg qd orally, while the intensive group received rosuvastatin 20 mg qd orally for six months. Results After 6 months, TC, TG and LDL-C levels decreased significantly compared with those before treat- ment, HDL-C levels increased significantly compared with those before treatment （P 〈 0.05 or P 〈 0.01） in two groups of patients, and the range of decrease or increase is more obvious in the intensive group than that in the general group（/） 〈 0.05）; and the plaque area and IMT were significantly reduced （P 〈 0.05） compared with those before treatment in the inten sive group, while there were no significant changes of plaque area and IMT （P 〉 0.05） in the general group. The plaque area and IMT were significantly reduced compared with those in the general group （P 〈 0.05） after treatment. Conclusion Rosuvastatin＇s intensive lipid-lowering function has a stronger lipid-lowering effect on ischemic stroke, through reducing platelet adhesion and aggregation, thereby reducing the plaque area and carotid IMT, which may block and reverse carotid artery plaque progress.
