乳腺癌患者携带PICC化疗并发导管相关性败血症的临床诊治探讨

摘　　要：目的:探讨乳腺癌患者携带经外周中心静脉置管(Peripherally inserted central catheters,PICC)化疗期间PICC导管相关性败血症的临床诊断和防治。方法:回顾性分析我院2009年8月-2011年9月携带PICC化疗的215例乳腺癌患者中PICC导管相关性败血症的发生及其诊治情况。结果:3例携带PICC化疗的乳腺癌患者分别在PICC置管后29、73 d和108 d(平均70 d)出现PICC导管相关性败血症,其发生率为1.39%,发病密度为0.16/PICC置管1 000 d(0.16‰)。结论:乳腺癌患者化疗期间携带PICC化疗是一种安全有效的方法。但少数病例有可能出现PICC导管相关性败血症,应注意观察相关症状和体征,早期发现、对临床表现疑似PICC导管相关性败血症患者不应拘泥于血培养结果,及时拔除PICC管,患者多能获得较好的疗效。Objective:To study the diagnosis and treatment for PICC-related sepsis in patients with breast cancer during chemotherapy.Methods:The data of the diagnosis and treatment for PICC-related sepsis in 215 patients with breast cancer during chemotherapy were retrospectively analyzed.Results:Three(1.39%) of the 216 PICCs were removed as a result of PICC-related sepsis in 29,73 and 108 catheter-days,with a rate of 0.16 per 1 000 catheter-days.Conclusion:Application of PICC in patients with breast cancer during chemotherapy is safe and effective.However some patients may develop catheter-related sepsis.In order to get a good therapeutic effect,we should pay attention to the symptoms and signs besides the blood culture results and treat it timely.
