膀胱良、恶性上皮肿瘤中△Np63的表达及临床意义

摘　　要：目的探讨p53基因家族成员p63的一种异构体△Np63在膀胱良、恶性上皮性肿瘤组织中的表达及意义。方法采用免疫组织化学SP法检测10例正常膀胱黏膜、8例内翻性乳头状瘤、50例膀胱癌(包括41例膀胱尿路上皮癌和9例膀胱鳞状细胞癌)中△Np63的表达,观察其在膀胱良恶性肿瘤中表达的差异,分析△Np63表达与膀胱癌病理类型及临床病理特征的关系。结果 (1)膀胱癌中△Np63的阳性表达率为80.00%(40/50),显著高于正常膀胱移行上皮10.00%(1/10)、内翻性乳头状瘤37.50%(3/8)2组(P<0.05);(2)高级别尿路上皮癌组织中△Np63阳性表达率显著高于低级别组,且其表达随浸润深度的增加而升高(P<0.05);(3)△Np63的表达与膀胱癌的临床分期密切相关(P<0.05);(4)△Np63在膀胱鳞状细胞癌中的阳性表达率较尿路上皮癌为高,但差异无统计学意义(P>0.05)。结论△Np63在膀胱癌组织中高表达,与膀胱癌组织分化、浸润程度、临床分期密切相关,其可能在膀胱恶性上皮肿瘤的发生发展过程中发挥重要作用,并可作为预后评估的重要指标之一。Objective To explore the expression of delta p63（△Np63）,a novel member of p53 gene family,in benign and malignant epithelial tumor of bladder and its clinical significance.Methods The immunohistochemical staining assay was conducted to detect expression of △Np63 in 10 cases of normal baldder epithelium, 8 cases of bladder inverted papilloma and 41 cases of bladder carcinoma, including 36 cases of uroepithelium carcinoma of bladder（BUC） and 5 cases of squamous cell carcinoma of bladder.We investigated the difference between the benign and malignant epithelial tumor of bladder and analyzed the relations among the expression of △Np63, pathological types and clinical stages.Results （1）The positive rate of expression of △Np63 in BUC 80.00%（40/50）was obviously higher than the normal tissue group 10.00%（1/10） and inverted papilloma group 37.50%（3/8）（P〈0.05）.（2）The positive rates of expression of △Np63 in high--grade BUC was significantly higher than in low--grade group.With the improvement of invasion tendency of BUC, the staining intensity of △Np63 was gradually increased （P〈0.05）.（3）The expression of △Np63 was correlated with clinical stages of BUC（P〈0.05）.（4）Higher expression rate of △Np63 was found in squamous cell carcinoma than in the BUC group,but there was no obvious difference（P〉0.05）.Conclusion △Np63 was highly expressed in bladder carcinoma,which was closely associated with the differentiation,invasion tendency,clinical stages of BUC.It may play a very important role in the occurrence and progression of BUC and become a potential factor for evaluating prognosis of BUC.
