23例复杂胫骨平台骨折手术疗效分析

摘　　要：[目的]总结复杂胫骨平台骨折手术治疗的临床结果,分析影响术后疗效的相关因素,探讨有效的防范措施。[方法]回顾分析某院2003年2月～2009年10月收治的23例复杂胫骨平台骨折手术后患者资料,其中男16例,女7例。骨折类型按Schatzker分型其中V型14例,VI型9例。治疗均采用手术开放复位、钢板螺钉内固定、塌陷缺损处自体骨植骨。对术后疗效及影响的相关因素进行分析。[结果]23例均获得随访,平均随访时间32个月(8～72个月),按照Rasmussen评分标准评价膝关节功能,优11例,良9例,可1例,差2例,优良率达到83.3%。[结论]外科手术是治疗复杂胫骨平台骨折十分有效的方法,注意手术时机的选择,术中骨折的解剖复位、稳定的内固定,术后注意伤口引流及关节功能康复治疗是影响患者术后疗效的重要因素。[Objective]To summarize the clinic experience of surgical treatment of the complex tibial plateau fractures,analyze the influencing factors for postoperative curative effect,and discuss effective preventive measures.[Methods]23 cases with complex tibial plateau fractures surgery in our hospital were collected from Feb.2003 to Oct.2009,including 16 males and 7 females.The 23 fractures were classified according to Schatzker classification：the 14 cases with type V and 9 cases with type Ⅵ.All patients were performed with open reduction surgery,internal fixation by plate and screw,autogenous bone graft on Collapse damage place.The postoperative curative effect and the influencing factors for the effect after surgery were analyzed.[Results]All the 23 cases were followed-up,and the average follow-up time was 32 months（8-72months）.According to Rasmussen Grading,the function of the injured knees showed excellent in 11 cases,good in 9 cases,fair in 1 case,and poor in 2 cases.The excellent and good rate was 83.3%.[Conclusion]Surgical operation is an effective measure to treat complex tibial plateau fractures.The decision of operation timing,anatomical reduction on fractures,steady internal fixation,nursing of the wound drainage and rehabilitation after surgery are important factors which will influence the postoperative curative effect.
