摘　　要：目的探讨自体骨髓单个核细胞移植对终末期肝病的治疗作用及不良反应。方法 22例终末期肝病患者,经骨髓动员后于髂前上嵴穿刺抽取骨髓液分离单个核细胞,采用介入技术经肝动脉植入肝脏中,观察治疗前、后肝脏代谢功能、合成功能、凝血功能和肝损伤指标的改变及不良反应。结果自体骨髓单个核细胞移植后随访24周,与治疗前比较,前白蛋白、白蛋白、胆碱酯酶、总胆红素、直接胆红素、总胆汁酸、凝血酶原时间、纤维蛋白原、丙氨酸氨基转移酶、门冬氨酸氨基转移酶、γ-谷氨酰转肽酶和碱性磷酸酶均无明显改善(P>0.05),治疗过程中无明显不良反应。结论自体骨髓单个核细胞移植对终末期肝病的疗效有限,尚需进一步改进。Objective To explore the therapeutic effect of autologous bone marrow mononuclear cell transplantation on end-stage liver disease and its adverse reaction.Methods Twenty-two patients with end-stage liver disease were harvested bone marrow suspension after bone marrow mobilization with granulocyte monocyte colony stimulating factor from anterior superior iliac spine under local anesthesia,then the mononuclear cells were enriched by density gradient centrifugation and were transplanted into the liver with hepatic artery intervention.The hepatic metabolic functions,synthetic functions,coagulation functions,damage indexes and the adverse reaction were evaluated before and after treatment.Results In a 24-week follow-up after the transplantation,prealbumin,albumin,cholinesterase,total bilirubin,direct bilirubin,total bile acid,prothrombin time,fibrinogen,alanine aminotransferase,aspartate aminotransferase,γ-glutamyltransferase and alkaline phosphatase were not improved obviously（P0.05）.Meanwhile no obvious adverse reaction were found.Conclusion The therapeutic effect of the autologous bone marrow mononuclear cells on end-stage liver diseases is limited and needs further study.
