摘  要:目的:探讨凝溶胶蛋白(Gelsolin)、P糖蛋白(P-gp)、拓扑异构酶Ⅱ(TopoⅡ)、谷胱苷肽转移酶(GST-π)在预测非小细胞肺癌(NSCLC)临床病理特征、癌细胞转移和患者预后中的价值。方法:采用S-P免疫组化法检测126例NSCLC患者癌组织标本及其癌旁组织标本Gelsolin蛋白、P-gp、TopoⅡ、GST-π表达情况。结果:Gelsolin蛋白在NSCLC患者中的阳性表达率为31.7%,P-gp为38.1%,TopoⅡ为52.4%,GST-π为85.7%。Kaplan-Meier生存分析显示,Gelsolin蛋白及P-gp阳性患者生存率较Gelsolin蛋白阴性患者为低(P<0.05),Cox风险比例模型显示淋巴结转移、Gelsolin蛋白及P-gp阳性可显著增加NSCLC患者的死亡风险(P<0.05)。结论:Gelsolin蛋白与P-gp表达增加与NSCLC淋巴结转移及肿瘤进展相关,联合检测Gelsolin、P-gp、TopoⅡ、GST-π,可作为临床早期发现NSCLC转移及预测预后的指标。Objective To investigate the expression of Gelsolin,P-gp,TopoⅡand GST-πin non-small cell lung cancer(NSCLC) and to determine the relationship between their expression and clinicopathological features of patients with non-small cell lung cacer.Methods SP immunohistochemical assay was used to detect the expression of Gelsolin,P-gp,TopoⅡand GST-π in 126 NSCLC and matched paracancerous tissue.Results The positive rate of Gelsolin,P-gp,TopoⅡand GST-πin lung carcinoma was 31.7%,38.1%,52.4% and 85.7% respectively.Kaplan-Meier survival analysis showed that Gelsolin or P-gp positive patients had shorter survival time than that with negative patients(P 0.05);Cox regression analysis revealed that lymph node metastasis or Gelsolin,P-gp positive expression were the noticeable affective factors on the death of the NSCLC patients after surgery.Conclusion Gelsolin and P-gp play an important role in the development in NSCLC and correlate with metastasis and shorter survival time in patients with NSCLC.Gelsolin and P-gp detections can be used as early biomarkers in metastasis and prognosis for NSCLC patients.
