摘 要:目的探讨抗精神病药所致迟发性运动障碍(TD)患者血浆非酶抗氧化物浓度变化与TD的关系。方法分别运用阳性和阴性症状量表(PANSS)和异常不自主运动量表(AIMS)评定精神症状和TD严重程度;使用全自动生化仪测定TD组(60例)、非TD组(60例)和对照组(45名)血浆白蛋白(Alb)、胆红素(Bil)、尿酸(UA)浓度,采用方差分析比较3组间上述指标的差异;采用单因素非条件Logistic回归分析各项生化指标与TD患者临床特征间的关系。结果与对照组[(46.8±3.4)g/L和(12.7±3.3)μmol/L]比较,TD组[(40.2±2.5)g/L、(9.2±2.1)μmol/L]和非TD组[(41.2±2.3)g/L、(10.2±3.1)μmol/L]Alb和Bil明显降低,差异有统计学意义(P均〈0.001);TD组Alb浓度低于非TD组(P=0.033),Bil浓度差异趋于显著水平(P=0.052),UA浓度差异无统计学意义;TD组AIMS总分与Bil(r=-0.373,P=0.008)、阴性症状分(r=-0.262,P=0.044)显著相关。结论非酶抗氧化物可能在TD的病生理机制中起一定作用。Objective To investigate the plasma non-enzyme antioxidants levels in chronic schizophrenic patients with tardive dyskinesia ( TD). Methods The plasma levels of albumin ( Alb ) , bilirubin (Bil) and uric acid (UA) in schizophrenic patients with TD (TD group, n = 60 ) and without TD ( non-TD group, n = 60 ) and healthy volunteers ( control group, n = 45 ) were assayed by using full automatic biochemical analysor. The plasma antioxidants levels were compared among three groups by one-way ANOVA. Psychopathological symptoms and severities of TD were assessed by using the Positive and Negative Symptom Scale(PANSS) and Abnormal Involuntary Movement Scale (AIMS) respectively, the relationships of the plasma antioxidants levels and the clinical traits of TD were analyzed. Results Plasma Alb and Bil in control group [ (46. 8 ± 3.4 ) g/L, ( 12. 7 ± 3.3 )μmol/L ] were significantly higher than those in TD [(40.2±2.5) g/L,(9.2 ±2.1) μmol/L] and non-TD [(41.2±2.3) g/L,(10.2 ±3.1)μmol/L] groups ( all P 〈 0. 001 ), but there were no significant differences on plasma UA concentrations among the three groups ( F = 1.42,P = 0. 234). There was significant difference on the Alb concentrations between the TD and non-TD group ( P = 0. 033 ), and trend significant difference on Bil levels ( P = 0. 052 ). In TD group ,the total score of AIMS were negatively related with Bil concentrations ( r = - 0. 373 ,P = 0. 008 ) and negative sub-score of PANSS ( r = - 0. 262, P = 0. 044). Conclusions The non-enzyme antioxidants might be involved in the pathopbysiology of TD.
